• Why were you there?
• How many members of staff were present/did you see?
• Were there other people/ young people in the area?
• What was the initial reason for the contact with the young person?

Approaching the Incident

PLEASE PROVIDE DETAILS OF YOUR APPROACH TO THE INCIDENT AFTER CONSIDERING THE FOLLOWING STATEMENTS

• What did you see?
• Where were you positioned?
• Proximity and positioning of other members of staff (if present)
• Describe if you or others were in/ perceived to be in imminent danger
• What was the young person(s) doing upon your arrival? e.g. threatening/ fighting/ stood over someone/ running away/ escaping, etc
• Did you see a weapon and if so, what type?
• Did you issue any verbal communication? What impact, if any did that have on the young person?
• What was your perception of what would have happened if you had not intervened?
• What did you do to de-escalate the situation?

Prior knowledge

PLEASE PROVIDE DETAILS OF YOUR PRIOR KNOWLEDGE OF THE YOUNG PERSON AFTER CONSIDERING THE FOLLOWING STATEMENTS

• Was the behaviour displayed by the young person common place or out of character?
• Were you aware if alcohol or drugs were an impact factor?
• Were you aware of any particular fighting skill that the young person held, or of recent, well known volatile history/ infamy?
• Were you aware of any mental impairment/ personality disorders of the young person?
• Details of any briefing given by the Supervising member of staff (planned incident)
• Quote any verbal responses made by the young person, if possible
• Describe any non verbal signs e.g. clenched fists/teeth/change of stance/staring/ intimidators & body posture etc
• Was there any increase in physical actions/ state of aggression?
• Their size, strength and presence compared to yours
• Was the young person in your or other’s personal space/ reactionary gap?
• How hostile were they and what level(s) of resistance were displayed?

Involvement

• What restraint techniques did you apply?
