  12. Extramural candidate must supply an official affidavit of cooperation with the Faculty of Medicine and Dentistry UP and recommendation letter of his/her dean or other superior of the institution (rector, director etc.).

Materials pertaining to items 1,6,7,8,9,10,11,(12) will be delivered in electronic form as well (these will be posted on the FMD UP website).

ad 6. Itemized structure of publications list

All parts of the list will contain the name of the candidate visibly marked (in bold type or by underlining)!!!

a)Original scientific contributions in the relevant field published in journals with impact factor (original contributions must contain original data, therefore reviews and abstracts are not included. State the impact factor for the journal in the particular year of publication.

b)Original scientific contributions in the relevant field published in peer-reviewed journals without impact factor.

c)Reviews in the relevant field published in journals with impact factor.

d)Reviews in the relevant field published in journals without impact factor.

e)Doctoral thesis (author, title of the thesis, place and date of defense)

ad 7.List of citations

Provide a list of citations that will clearly identify which publication of the candidate is cited by whom. All authors of the cited work and all authors of the citing must be listed. Citations included in the list must not contain any of the authors of the cited work. Citations found in SCI database (Web of Science) must be clearly identified so.

ad 12.Habilitation thesis

a)Habilitation thesis must contain new scientific knowledge.

b)Should the habilitation thesis be submitted as a compilation of previous publications, then

ba)it must consist of majority of publications published in peer-reviewed journals,

bb)Must contain written affidavit of co-authors stating the candidate´s contribution.

c)If a book is submitted as the habilitation thesis, then the candidate must provide written affidavit of all co-authors stating the candidate´s contribution.
