Please complete the following form in full in order to be considered for funding. The form must be completed and returned electronically in Microsoft Word format to Daniel Waclaw, Associate Director, at no later than 5:00 p.m. Central time on Wednesday, March 2, 2016. Purpose of Seeking Women’s Board Funding

Applicants must make a clear and convincing case about the specific strategic value that a grant from the Women’s Board will bring to the proposed project. Check one of the following three purposes that best fits your reasons for seeking Women’s Board funding. Please describe your reasoning in greater detail in Part II.

Catalyst. Funding serves as seed money or innovation capital.
Sustaining. Funding supports continuation of successful, proven efforts for which inadequate other support exists.
Capacity-Building. Funding covers one-time expenses that will enhance existing activities in a significant way.

Part II: In-Depth Proposal Description

Please use the remainder of the following two pages to explain your proposal in specific yet not highly technical language. (You may delete this list of questions to give yourself more room to write if necessary.) Be sure to cover the following questions:

  • Who will be served by this project? What is the impact of this project on the University community? How will this further the University’s mission?
  • How will funds from the Women’s Board be used in support of this project?
  • What other sources of funding are committed or are being sought after for this project? Could this project go forward without Women’s Board support?
  • Will this project be multi-year? How do you anticipate the project will be funded in future years? Will Women’s Board funding last for more than one year?

Part IV: Engagement Opportunities and Follow-Up Requirements

Engagement Opportunities for Women’s Board Membership

Applicants must include any opportunities for the Women’s Board membership to either directly or indirectly interact with the proposed project. Examples of engagement opportunities might include an invitation to an opening reception, access to speaking engagements, websites or printed publications featuring results from the project, etc. Please use the following space to detail any planned, proposed, or potential engagement opportunities for your project, assuming it will be funded by the Women’s Board.

Checklist of Follow-up Requirements for Grant Applicants

Please initial next to each of the following items in order to be considered for funding. If your project is selected for funding, you must adhere to these four commitments.

I understand that I must give an update on the progress of my project twice a year when prompted by Women’s Board staff (in autumn and spring) by using the appropriate form.
I understand that I must provide at least one high-resolution photograph per update that can be used for Women’s Board purposes. Please send photography of the project or primary investigator in action. This will be included in annual appeals and other communications to Women’s Board membership.
I understand that I must report any media mentions regarding my project immediately to the Women’s Board staff.
I understand that I must mention the University of Chicago Women’s Board by name when communicating with media about the project or developing communications detailing the project to the public.

PDF format is not accepted for the Grants Fund application form

  • Letter(s) of endorsement in PDF format
    Letter(s) must be signed by an accepted signer. Please review our website at http://womensboard.uchicago.edu/approved-signers.

Optional:

  • Applicants may also include supplemental materials (e.g., reports, research findings, brochures, presentations, articles, links to online content, etc.), but such materials must be bundled together as one separate PDF file.
