United Way’s Contract Year

  • Select the correct quarter (1st, 2nd, 3rd, or 4th) that is being reported on: Only one box should be selected. To select or unselect a box, double-click on the box. A prompter will appear. Under default value option, select either “checked” or “not checked.” Select “OK.”

Program Capacity Report

  • Contract Year Target: This number represents how many new clients/units are expected to access the program throughout the entire fiscal year. This number remains the same for every quarterly report. It is possible that a program has multiple units as targets. Note: Please specify year target using the unit that is appropriate for the population being served or the service being provided. Example: 50 families, 80 individuals, 45 parents, 100 youth, 2000 rides, etc…

Clients/Units Served – Unduplicated “New” Numbers:

Refers to clients/units new to the program that: (a.) began services during the current fiscal year (July 1, 2009 – June 30, 2010) and (b.)have not been recorded in previous reports.

  • New number served this quarter: Report how many unduplicated new clients accessed the program during the quarter. These clients have not been recorded in any previous report.
  • New number served year-to-date: Report how many unduplicated new clients have accessed the program since the start of the contract year. This is a cumulative number that builds upon the previous quarterly report.

Clients/Units Served – Duplicated Numbers:

Refers to clients/unitsthat are (a.) notnew to the program and (b.) continue to access services.

  • Total number served this quarter: Report how many clients/units (duplicated and unduplicated) accessed the program during the quarter.
  • Total number served year-to-date: Report how many clients/units (duplicated and unduplicated) have accessed the program since the start of the contract year. This is a cumulative number that builds upon the previous quarterly report.

Outcome Report

In this section, only report on numbers related to the target outcomes. These numbers may represent the entire population served by the program or just a subset of the population.

  • Insert Outcome: Enter each outcome exactly as it was stated in the Outcome Measurement Framework. This isnot an Indicator, and does not include numbers. Note: For Outcome #1, use the outcome that corresponds to the Influencing Factor represented in the Outcome Measurement Framework.
  • Total number served:Number of clients/units served this quarter that could be measured for the outcome. Note: Only report on clients/units that are appropriate to the outcome. Also, be sure to use the specific unit that applies to the outcome being measured. Example: 50 families, 45 parents, 200 rides, etc…
  • Total number reported: Number of clients/units included in the results of the outcome.
  • If the total reported is not the same as the number served, please describe why: In most cases, “total number reported” should match “total number served.” However, for some programs it may not always be possible to report on every client served. Possible reasons these numbers may differ: (a.) client did not return survey; (b.) inability to gather data due to early termination; (c.) client has not been in program long enough to gather data; etc…
  • Insert Indicator: Type in each Indicator where requested. Each Indicator should be entered exactly as it was stated in the Outcome Measurement Framework.
  • This Quarter:Identify numbers that did or did not achieve outcome during the quarter.

Number reported – Number of clients/units included in results. This number should be the same as the “total number reported” for the outcome.

Number achieved – Number of clients/units who achieved the anticipated outcome.

Percent achieved – Percentage of clients/units who achieved the anticipated outcome. To get this number, divide the “number achieved” by the “number reported.”

Number achieved ÷ Number reported = Percent achieved.

  • Outcome Target:Percentage of client/units anticipated to achieve the outcome.For each outcome, this is the same percentage number stated in the Indicator. This number remains the same for each quarterly report.
  • Year to Date:(IMPORTANT NOTE FOR “NUMBER REPORTED”)

Number reported – Number of clients/units included in results since start of the contract year. This is not a cumulative number. Only report a client/unit once in this box. If a client has been reported in this box for a previous quarterly report, do no count the client a second time. Doing so will produce an invalid Year to Date Percentage.

Number achieved – Number of clients/units who achieved the anticipated outcome since start of the contract year. This is a cumulative number that builds upon the previous quarterly report. Each quarter will indicate ALL clients who achieved this outcome for the year.

Percent achieved – Percentage of clients/units who achieved the anticipated outcome. This is not a cumulative number. To get this number, divide the “number achieved year to date” by the“number reported year to date.”

Number achieved YTD ÷ Numberreported YTD = Percent achieved YTD

  • How the target was determined: Identify any data that supports the outcome target. Be as specificas possible, and cite any applicable references. Example: the target is based on: (a.) the program’s past performance, (b.) national data/statistics, (c.) a best estimate, etc…
  • Interpretation of Results: Highlight any successes and/or challenges specific to the outcome results. Discuss any conclusions that may be drawn from the data. Are the outcome results consistent with the anticipated target results? If not, offer a brief explanation as to why not. Consider all factors that may have an impact. Note: For Outcome #1, make sure to discuss the specific Influencing Factor identified in the Outcome Measurement Framework.
  • Action Taken:If applicable, explain how the program has responded to any challenges identified in the “Interpretation of Results.” Specifically discuss any current and/or future changes to the program that are a response to identified barriers. Example: adopting a different system for data collection; introducing new program services; changing the target population being served; etc…
  • Influencing Factor: Influencing Factors can be tracked quarterly or cumulatively. On the reporting form, indicate whether the program is using “This Quarter” numbers or “Year to Date” numbers. Once the program has decided how the numbers are reported, it is important to consistent. Report the same way each quarter. (Only required for Outcome #1)

Insert Influencing Factor: Type in the Influencing Factor where requested. This should be entered exactly as it was stated in the Outcome Measurement Framework.

Insert Indicator: Type in the Indicator where requested. This should be the same Indicator used for Outcome #1 and entered exactly as it was stated in the Outcome Measurement Framework.

Total Achieved/Not Achieved: Separate the “number reported” in Outcome # 1 into two groups: Achieved Outcome vs. Did Not Achieve Outcome. Insert those numbers in their respective boxes. Note: Number achieved here is the sameas “number achieved” stated in the Outcome # 1 Indicator box.

Percent Achieved/Not Achieved:

Number achieved ÷ Number reported = Percent achieved

Number not achieved ÷ Number reported = Percent not achieved

Insert Category A/B: Categorize Influencing Factor into two or more groups. List these categories in their respective boxes. Example: (Age) Under 18 Years Old /18 & Older; (Sex) Male/ Female; (Country of Origin) Born in USA /Born Outside of USA.

Category A & B Achieved/Not Achieved: Separate each category into two groups: Achieved Outcome vs. Did Not Achieve Outcome. Insert those numbers in their respective boxes. Follow the above formulas to report the percentages for these categories.
