JOB DESCRIPTION

OUTREACH PT/OT

Location : Wayil, Ganderbal

Mandate:

Under the direct supervision and responsibility of the Project Coordinator, and the overall management of the Executive Director, the OUTREACH PT/OT will be based in the assigned district.

The OUTREACH PT/OT will work under the outreach mode of HDC under project support structure, committed to provide reliable and compliant support to program activities in order to achieve efficient service to program beneficiaries.The OUTREACH PT/OT’s mandate is to put in place a proper and comprehensive client circulation/management system in the assigned district, including all components of the rehabilitation services (therapy, training, follow up etc.).

Responsibilities:

Production and services:

  • Support the Project Coordinator, Hope Disability Centre to set up the rapport building with relevant stakeholders, within the assigned district in order to increase the footprint of Hope Disability Centre thereby providing the Community Based rehabilitation services in the district.
  • Manage the activities of the project and ensure that all actions are undertaken in an organized and systematic manner as per the agreed work plans in the assigned district.
  • Organize assessment camps and rehabilitation camps to identify beneficiaries and provide basic rehabilitation services in communities.
  • Implement community based rehabilitation services to provide quality and comprehensive rehabilitation care to people with disabilities.
  • Assess all clients, develop/implement interventional plan including goals ( Long term and Short term) for each and every individual client
  • Train the client and caregiver on necessary physical rehabilitation services which he/she needs to follow to enhance the improvement
  • Ensure the client files are regularly updated with all key information
  • Deliver Aids and appliances to clients after proper assessment and need. Train the client/caregiver on the use and maintenance of aids and appliances.
  • Identify clients who need P&O services and link them with P&O unit or organize assessment and donation camps
  • Support in recruitment of volunteers if needed and building their capacities to develop CBR services.
  • Ensure that different stake holders are in sync with the project objectives and goals and understand their responsibilities for supporting the smooth and effective implementation of the project.
  • Under the leadership of the Project Coordinator, support, follow-up and supervise the work of the Assistant Outreach PT/OT in the targeted areas.
  • Focus on building the capacities of Assistant Outreach PT/OT for ensuring smooth services in terms of rehabilitation, care, and also information sharing.
  • Coordinate with the appropriate team members to ensure the subordinates are receiving the required support to identify people with disabilities and their families, assess their needs and to ensure these means are provided; organize and implement trainings when required with the support of
  • Facilitate the referrals for specialized care through the referral system to the Physical Rehabilitation Centre, medical institutions and any other institution for beneficiaries within target communities.
  • Support the organization of disability certificate camps.
  • Support in the formation of Self Help Groups of caregivers of people with disabilities and to facilitate them to provide peer support and create awareness in disability issues.
  • Support in organising training programme for Community Workers and for Integrated Child Development Scheme (ICDS), National Rural Health Mission (NRHM).
  • Assist in the facilitating training sessions and awareness materials on rehabilitation and disability sectors.
  • Travel to the allocated communities to implement all activities and interact with the relevant stakeholders of the project.
  • Come to the Hope Disability Centre headquarters in Wayil minimum once in a month to meet with managers.
  • Other activities related to project implementation as per the needs
  • Support the team in organizing community awareness sessions and surveys

Reporting

  • Maintain accurate records of activities and assessments conducted, support and services provided, and referrals made, as well as stocks available within the assigned district.
  • Ensure that the Assistant PT/OT maintain records of activities and statistics and compile the different reports.
  • Write regular statistics, activity and progress reports and provide them to the Project Coordinator in timely manner.
  • Write case studies on beneficiaries and activities of the project if required.
  • To ensure that all donations made from the field are accompanied with a signed donation certificate.
  • Ensure all services are provided in a professional and ethical manner.
  • Communicate with other staff to ensure that the beneficiaries receive services and that continuity of care occurs.
  • Use various communication means (written, spoken, etc) during activities to ensure effective inclusion of all participants.
  • Maintain a positive image of Hope Disability at all times and in all circumstances.
  • Support the Project Coordinator in carrying out monitoring and evaluation of the project activities to ensure expected results are being reached and activities are implemented according to the work plan
  • Regularly update the data of all project services as described

Supervisory responsibility

This position would be directly responsible for managing the rehabilitation services at assigned district and will be directly supervising one Assistant Outreach PT/OT.

Education, Knowledge, Experience & Skills

  • Minimum 3 yrs experience in a Physical Rehabilitation Centre/outreach.
  • Masters or Bachelor’s Degree Rehabilitation Services like, PT, OT.
  • Good training skills
  • Good organization & adaptive skills
  • High level of flexibility and strong interpersonal skills
  • Willingness for Frequent travel to remote areas
  • A good level of written and spoken Urdu and English and spoken Kashmiri is essential.
  • Motivated to work in a NGO setting and to learn.
  • Motivated to work in the profession and to help people, particularly PwDs
  • Good verbal and written abilities as well as the capacity to interact with the different kind of stakeholders (families, volunteers, local government officials).
  • Able to work in a team and under supervision.
