I have read and understand the procedures for academic appeals as outlined in Part 57 of the Texas A&M University Student Rules. I further understand that this Preliminary Questionnaire and all pertinent documentation must be filed within one semester after the decision being appealed was made. The act of filing this Preliminary Questionnaire is construed as authorizing all panel members to have full access to all records, including academic, civil and medical records that may have a bearing on deliberations. I also understand that my failure to appear at the deliberations without justifiable cause shall terminate my right to appeal.

Provide a statement indicating the basis for the academic appeal and list the evidence that you will present to support your appeal. Note: all evidence and supporting documents and short statements from any witnesses must be attached to this Preliminary Questionnaire. Documents not included in this request for a grade appeal may not be admitted into evidence during the deliberations of the Undergraduate Academic Appeals Panel. Contact information for all witnesses must be included as a part of their written statements.

Members of the Undergraduate Academic Appeals Panel may question the student, department head, dean (or his or her designee) and witnesses during the proceedings. An audio recording of the statements, questions and answers will be made during the open session of the Undergraduate Academic Appeals Panel proceedings.

Please note that the burden of proof shall be upon the student. The judgment made by the Undergraduate Academic Appeals Panel will be made based on the preponderance of evidence. The dean (or his or her designee) will be invited to be present during the proceedings.
