The following scholarship will be awarded to graduating UIC School of Public Health students (including off-campus and international students) that have or will complete their studies in academic year 2017-2018 (Summer 2017, Fall 2017, Spring 2018). Applications will be reviewed and award recipients selected by the Committee on Academic Progress (CAP).

Alan W. Donaldson Award

AY 2017-2018

Description of Award: The Alan W. Donaldson Award honors one graduating student for significant academic excellence, qualities of leadership, and community service. It is the most prestigious award granted by the UIC School of Public Health and is a memorial to Alan W. Donaldson, ScD, a founding father of the School and its first Associate Dean. The winning student will be required to speak at the annual Spring Commencement on Friday, May 5, 2017.

Award Amount: $1,500

Eligibility: Applicants must be among the graduating class from a School of Public Health degree program. Additionally, applicants must be available to speak at the School’s Commencement.

Scholarship Criteria:

·  Students must demonstrate significant achievements in the following areas:

o  Academic Excellence: Academic excellence is evaluated based on cumulative grade point average (GPA). Other indices of academic excellence include a strong record of research achievement evidenced by active participation in seminars and scientific conferences, publications, and/or a high-quality thesis, dissertation, or MPH essay, etc.

o  Leadership: The applicant should demonstrate distinguished leadership and excellence among his or her peers. Evidence for this criterion includes: playing a guiding role on student or community committees, initiating and facilitating projects that make significant contributions to the quality of life (academic and social) at the School of Public Health and/or within the larger community, promoting programs leading to positive change within or outside the School, and acting as a mentor or role model for one’s peers.

o  Community Service: The applicant should contribute significantly to community or service organizations outside of the university. Activities may include volunteer work, initiating or organizing activities, providing services and taking on administrative responsibilities. Evidence of community service is not limited to contributions made during the applicant’s student career. The applicant’s lifetime achievements will be considered.

·  Copy of student transcripts – may be printed from My UIC

·  Personal Statement (not to exceed three pages) – Must provide descriptions of your academic excellence, leadership and community service efforts as outlined above

·  Two letters of support from SPH faculty members

·  One letter of support from an outside community agency or organization

·  Current CV/Resume

Application Deadline: Thursday, March 1, 2018 by 4pm; late submissions will not be accepted.
