Please complete the following questionnaire. I appreciate that this will take some time and hope you will answer each question thoughtfully and as fully as you can. Please note that there is not necessarily a right or wrong answer to questions. Your answers will help frame our future discussions about a puppy and puppy care and will help me determine the most appropriate puppy for you and your family. They will also help me to provide you support in preparing to receive your puppy and in caring for him or her over the long term. You may type in your answers in the document after each question.
