** Note: Anyone interested in coaching a Travel Team must have a letter of interest submitted to the TRIC Athletic Board by 3/29/10. A background check will be conducted on all coaches/asst coaches.

Fees and Registration Information: $25 per participant for the Pre-K league(limited schedule). $40 per participant; $70maximum for families with 2 or more children participating in our regular leagues. Please mail forms/fees to TRIC Athletics P.O. Box 69, CartervilleIL 62918 .Make checks payable to Tri-C Athletics. Forms/fees will also be collected at the Junior High Multipurpose Room on 3/6 and 3/13 between the hours of 9:00 am and noon 3/16 and 3/18between the hours of 5:30-7:30pm. No registrations will be accepted after Tuesday,March 23, 2010. Siblings will be placed on the same team where age guidelines allow unless requested otherwise. No other team placements will be guaranteed. Please indicate the siblings to be placed on same team. ______

**Any participant who has played 2 consecutive years within the same TRIC league according to our recordshas the option of moving up to the next league regardless of the age guidelines. TRI-C ATHLETICS

PARENT/GUARDIAN CODE OF CONDUCT

As parent/legal guardian of a child involved in the Tri-C Athletics Program, I agree to abide by the following Code of Conduct. I resolve to:

  1. Promote sportsmanship by demonstrating positive support for all players, coaches, opponents, spectators, and officials at games, practices and other Tri-C activities.
  2. Place the emotional and physical well-being of my child before any personal desire I may have for my child to win—always remembering that games are meant to be fun, and children participate to have fun.
  3. Encourage my child to play by the rules and respect the rights of other players, coaches, opponents, spectators and officials. It is strongly encouraged that all parents attend practices and games of their children.
  4. Respect the officials and their authority during games. I will NOT engage in behavior that is critical, distracting, or derogatory toward an official (including any form of verbal or physical assault). I realize that sports involve decisions and judgment calls, and I will respect those decisions and teach my child to do the same.
  5. Refrain from coaching my child or other players during games and practices, unless I am one of the official coaches. I will remain in the stands, away from the playing field and player benches, unless requested to provide assistance by a coach or official.
  6. Praise my child for competing fairly and trying hard—and I will NOT ridicule my child or other participants for making a mistake or losing a competition.
  7. Support my child’s commitment to the team by assuring regular attendance at practices and games. I will make a commitment to have my child on-time for events, and picked-up promptly at the conclusion of an event.
  8. Review and become familiar with Tri-C Athletics sporting event rules.
  9. Respect all playing facilities (fields, gymnasiums, city property, etc) and strive to keep them clean. This includes promptly removing personal trash and debris, and parking only in approved areas.
  10. Support and appreciate volunteer coaches, officials and Tri-C Board members. I realize that without their efforts, my child would not be able to participate.

I agree that if I fail to abide by the aforementioned rules and guidelines, I will be subject to disciplinary actions by the Tri-C Board. These actions could include, but are not limited to, the following actions by an official, coach, and/or Tri-C league representative: (1) verbal warning, (2) ejection from the event with written documentation or the incident to be kept on file with the Tri-C Board and other organizations involved, (3) one game suspension instituted by the Tri-C Board, (4) season suspension instituted by the Tri-C Board, or (5) the authorities being notified if refusal to abide by an ejection from an event.

I/we, parent(s) of the above named youth, hereby give my/our approval for the above named youth to participate in all league activities during the current season. I/we assume all risks and hazards incidental to each child’s participation and I/we do hereby waive, release, absolve, indemnify and agree to hold harmless the local league and any of its affiliates (sponsors, national franchises, supervisors) for any claim arising out of an injury to the above named youth. I/we will furnish a certified birth certificate of the above candidate upon request.
