I am pleased to confirm your conversation with Heather Williams regarding your verbal acceptance of the offer of employment with DecisionOne Corporation.

This is your opportunity to become an integral part of DecisionOne which serves leading companies and government agencies with tailored information technology support services that maximize the return on IT investments, minimize capital and infrastructure costs, and improve operational effectiveness. Travis, we believe your skills and experience will benefit DecisionOne and help our company achieve its business goals.

You are tentatively scheduled to continue your career with DecisionOne in the position of Technician-Help Desk 1 Support at an hourly rate of $12.29. You will be reporting to Heather Williams. Please report to Heather Williams on your first day of employment. Your normal work hours will be determined between you and your manager. Please be advised that due to business needs, these hours may change.

This offer is contingent upon successfully completing and faxing back ALL required employment documents prior to your first day of employment, in addition to a successful background check and preemployment drug screening. Please be advised if your required employment documents are not received and verified by Human Resources, you may not begin working for DecisionOne on 10/18/2007.

Please make sure you arrange to meet with Heather Williams or go to a local DecisionOne office to have the U.S. Department of Justice I-9 Form successfully completed. You can have the required employment documents faxed by the local DecisionOne office and please make sure that you retain and mail the originals to Human Resources directly. Please fax to my attention at 610-993-6334. Your required employment documents consist of the following; (1) Business Use of Personal Motor Vehicle (2) Employee Confidentiality Agreement (3) Acknowledgement Form (4) Conflict of Interest (5) Employment Application (6) Authorization to Procure a Consumer Report (7) Hazard Communication Acknowledgement page (8) I-9 fully completed by DecisionOne Manager or Branch Office with copies of identification provided (9) Self Identification Form & (10) Signed Offer Letter.

PLEASE NOTE: If the automated response indicates that this is an Independent Third Party Collection Site please press #2 for another Collection Site. If all the locations given on the automated system are Independent Third Party Collection Sites, please call me for further details regarding an authorized Quest Diagnostics Lab in your area. Independent Third Party Collection Sites are not valid labs for pre-employment drug screening. You will need to take the Forensic Testing Custody and Control Form found in the new hire packet to the lab along with a picture ID within three (3) days of receipt of this offer letter. DecisionOne will incur the cost for this process. The background check may include, but is not limited to, employment verification, credit history, and felony criminal search. Enclosed is a Summary of Your Rights under the Fair Credit Reporting Act (FCRA), Disclosure and Notification letter, and an Authorization for DecisionOne to Procure a Consumer Report form.

Benefits

Because you were hired within 12 months of your termination date as a regular employee scheduled to work 25 hours or more per week, you and your eligible dependents will be eligible for DecisionOne benefits effective your first day of active employment.

Within four weeks after your first day of employment you will receive a benefits enrollment package delivered to your home. If you do not receive a package within this timeframe, please contact the Benefits Hotline at 800-860-1647 (Prompt #5). The benefits package details the specific benefits and cost that apply to you. Carefully read the benefits information and select the benefit that best meets your needs. This may include waiving certain benefits that you do not desire.

You will automatically be given default coverage for Basic Life Insurance, Basic AD&D Insurance and Long Term Disability Insurance at 40% of your base salary, if you do not select your benefits within the timeframe specified in your benefit package. This coverage will continue until your first opportunity to make a change during the next annual benefits open enrollment period. Your signature on this document acknowledges that you understand that if you do not elect benefits within the timeframe outlined in your benefit package or contact DecisionOne within four weeks from your hire date if you haven’t received a benefits package at your home address that you will be assigned the default coverage.

The benefits package offered at DecisionOne qualifies under the IRS Code Section 125 as a Cafeteria Plan allowing the premiums you pay, through payroll deduction, to be withheld on a before-tax basis.

Please be advised that neither this offer of employment, nor any statements made by any officer or employee of DecisionOne constitutes an employment contract, and that you are an “at-will” employee of DecisionOne.

Please make sure you fax all required employment documents to my attention immediately so the documents can be verified. If your employment documents are not received & verified, you will not be able to begin working for DecisionOne.
