Transition Plan

All active clients must be discharged from a site which is being closed and send as a continuing care to another site (other site can be a new or existing site). All clients transferred as a continuing care must be admitted at receiving agency.


PART A

Steps to transfer the current active clients under “Current Site (to be closed)” to appear under New site in after site is created may be within agency or different agency)

1)  Log in as Agency User

2)  Select clients under Admission module to discharge and click on discharge button:-

3)  Click on “Yes” when discharge confirmation window pops-up.

NOTE : Active authorization for the client (if exists) will be cancelled automatically, affective from the discharge date.

4)  Please enter all required fields indicated by red asterisks. Make sure to enter discharge date as last treatment date at closing site if the client is continued taking treatment at new site.

5)  At “Reason for discharge”:-

Select “Treatment Plan not completed”

6)  Select the Treatment plan not completed reason, select either “Site license Voided” or “Site moved to parent agency”.

7)  Select option for “Continuing care coordinated with new agency”, if current site license is voided and the new site is under different agency


8)  Click “Save” button and complete remaining part of discharge module, to make sure client is completely discharged.

PART B

Steps to admit the continuing care clients by receiving agency

1)  Login as the Agency where the new site is created/Agency where clients have been sent

2)  Click on “Parallel/Continue” button from left side navigation.

3)  Click on “Continuing Care” button, all continuing care clients must be displayed.


NJSAMS Admission Procedure

4)  Click on “Add to DASIE” link at the right most end by the client under “Continuing Care”, the client’s DASIE which was done at previous agency will be populated and need to review

5)  Immediate Need Profile (INP)

6)  UNCOPE – optional (once enter one question need to enter for all questions)

7)  Income Eligibility and Program Eligibility (Populated from previous – Editable according to current status)

8)  ASI and BioPsychoSocial (optional)

9)  Complete DSM-5 (mandatory module) – If the client to do admission in NJSAMS the client must be diagnosed. If “No Diagnosis” (or) “At Risk” can be continue to LOCI but no admission is allowed.

10)  Complete LOCI-3 (mandatory module) – If “No Diagnosis” (or) “”At Risk” in DSM-5, Recommended Level of care can select only 0.5 Early Intervention and there will be no “Admission” link at left navigation.

11)  Please check Agency recommended level of care in LOCI summary before continue to admission

12)  Complete the Admission module and obtain new authorization if applicable.
