To be completed by all participants. If participant is under 18 years of age, both participant and guardian must complete this release. Participant and guardian must sign in the presence of two witnesses.
TO SOUTHEASTERN LOUISIANAUNIVERSITY:
Participant, ______, and guardian understand that participant will participate in the Swamp Bowl Tournament to be held on the campus of the Southeastern Louisiana University on Friday, April 19, 2013. Participant and guardian understand that Swamp Bowl is a regulation volleyball game played in mud. Participant and guardian understand that participation in the tournament is voluntary and not required by the SoutheasternLouisianaUniversity.
In consideration of SoutheasternLouisianaUniversity permitting participant the opportunity to participate in this activity, participant and guardian, in full recognition and appreciation of any risks, hazards or dangers that may be inherent in this activity, do hereby agree to assume all of the risks and responsibilities surrounding participation in such activity. Further, I do for myself, my heirs and personal representatives, hereby defend, hold harmless and indemnify, release and forever discharge Southeastern Louisiana University, its trustees, directors, officers, agents, servants and employees from and against any and all claims, demands and actions or causes of action on account of damage to personal property or personal injury or death, which result from my participation and which result from causes beyond the control of, and without the fault or negligence of, Southeastern Louisiana University its trustees, directors, officers, agents, servants and employees, during the period of participation as aforesaid.
Participant and guardian fully understand the risks involved in this activity, and agree to assume those risks. Participant and guardian understand that SoutheasternLouisianaUniversity, its trustees, officers, agents, servants and employees assume and accept no liability for personal injury, loss of life or damage to personal property.
I, as participant, and my guardian (where applicable), attest and verify that I am physically fit and have sufficiently trained for participation in and completion of this event.

WEDNESDAY, APRIL 17MANDATORY CAPTAINS’ MEETIN, BALL ROOM
@ 5:00 p.m.Every team’s captain or a representative must be
present. Teams will be automatically disqualified if
a team representative is not present at meeting.
This meeting will be held at 5:00 p.m. at the Student Union Ballroom. Captains (or representatives) will pick up their teams’ playing time and t-shirts.
FRIDAY APRIL 19 SWAMP BOWL
Games begin TBAin front of the Pennington Student Activity Center. Team must report 30 minutes prior to the first scheduled game. The game will be forfeited if all team members are not present at game time.
