The 2009 Southeastern Dairy Youth Retreat will be held on July 5-8, 2009 at Virginia Tech in Blacksburg, VA. This event is being hosted by the Virginia Cooperative Extension Service.

In order for you to attend this retreat, you must complete the 5 enclosed registration forms and provide a $200 check made payable to the “Virginia 4-H Foundation – Dairy Account” for the registration fee as noted below:

1. Check for $200 made payable to the Virginia 4-H Foundation – Dairy Account (registration fee)

2. 2009 Southeastern Dairy Youth Retreat Registration Form

3. North Carolina Code of Conduct Form

4. North Carolina Medical Release Form (NOTE: This form must be notarized).

5. Virginia 4-H Code of Conduct Form and Medication Form

6. Virginia Health History Report Form

IMPORTANT:

Send the 5 completed forms and $200 check (made payable to the “Virginia 4-H Foundation – Dairy Account” ) to Dr. Brinton A. Hopkins at NC State University

by Wednesday June 17 at the following address:

REGISTRATION COST:

The registration cost of this retreat has been set by the organizers in Virginia at $200 per person.

TRANSPORTATION ARRANGEMENTS AND COST:

Once we receive your registration forms, we will contact you to let you know the travel arrangements and if there are any additional travel charges.

The North Carolina Dairy Youth Foundation and the North Carolina PDCA are providing financial support to help offset the travel costs for our North Carolina youth. However, in addition, there may be an additional charge for transportation for our North Carolina group to travel to this retreat.
