Re: Testing for Special Needs Students–LOCATION AND PROCEDURES

Our site is located in the SagtikosBuilding, Room 101A(where student exams need to be dropped off and picked up).

In accordance with the American with Disabilities Act (ADA) and the Rehabilitation Act of 1973, Section 504, the student will have documented his/her disability and is legally entitled to services and accommodations that are identified inthis letter. After the student has shown the faculty member the letter he/she should work with the professor to assure the following:

Student Responsibilities
A student with special needs must provide each professor with a Confidential/Special Needs Form generated by the CounselingCenter at the beginning of the semester. This form will specify the student’s special needs and requirements (it is confidential and must not be discussed in class).

  • Schedule the exam with the AcademicSkillsCenterat least one week in advance.
  • Notify his/her instructor of the date and time the exam has been scheduled.
  • Arriveon time for the scheduled appointmentand take the exam; if the student has to cancel, he/she shouldnotify the AcademicSkillsCenterand instructor before the appointment.
  • Notify the instructor after the exam has been taken.

Professor Responsibilities

  • Complete the “Exam Proctoring Sheet” available in Sagtikos101A (the special needs testing office) or in 100whenthe exam is dropped off. All examsmust beeither dropped off at the AcademicSkillsCenter ore-mailed (with the completed “Exam Proctoring Sheet” attached) at least two days prior to the scheduled exam date.The e-mail address is . A staff member will reply confirming receipt of the test.
  • If there is a situation that will not allow for delivery of a test as described, please contact one of the contact individuals listed below to coordinate arrangements for the test delivery (2) two days prior to test taking date.
  • Students are not allowed to deliver their own exams to the skills center (as in the past). If this takes place the student will be sent back to the faculty member and asked to follow the proper protocols. This action is taken to prevent loss of integrity and secure the exam’s safety.
  • Please encourage students to make appointments for tests at least one week in advance, and arrive for scheduled appointments on time. If they missan appointment, they will be required to reschedule the appointment with your approval.
  • Upon the student’s completion of an exam,you may pick up the exam during ourregularly scheduled hours at the AcademicSkillsCenter, Sagtikos 101A or 100.
  • Security of exams and student confidentiality is our foremost concern. The AcademicSkillsCenter maintains a secured, locked, testing file for each student with special needs. When an exam is received, it will be placed in his/her file in a locked cabinet in our locked office. ONLY Qualified personnel proctor students, and testing will be done in one of our testing rooms. TheAcademicSkillsCenterwill provide adequate security for all exams and ensure that students receive the testing accommodations they are legally entitled to receive.
