I authorize the Caputo Historical Families Association to utilize all public means available to verify the information contained herein. I understand "The Association" will not contact any given references without my direct consent. In accordance with the Common Law, your electronic signature constitutes a legal and binding application agreement.

No person, member or other, is allowed to send, bestow or grant any Diplomas, Letters of Patent or any Certificates in the name of the Organization. Offices and awards within the organization have to be granted by the President of The Caputo Historical Families Association, with all seals, and signatures.

All awards that can be achieved through The Caputo Historical Families Association can only be offered to worthy individuals with a good reputation. We cannot and do not accept every person who is proposed or applies for receiving an award by the Association. The Council will examine the background of each candidate.

Instructions for Completing the Application for Membership of

The Caputo Historical Families Association

Complete the application fully by providing your full name, address, telephone and e-mail address.

When this is complete you will need to list any Orders you belong to, titles of Nobility you may have, and any degrees you may have received. Please note that if there is not enough space on the application to complete these sections you may attach additional pages with the information or if the information is already on your CV or Bio just state on the application to “Reference attached CV.”

Any Chivalric Orders noted and titles of Nobility received must be from an approved fons. Academic degrees must be from an established accredited or foreign equivalent institution. Religious degrees that may not be accredited but awarded by a Church, recognized by that Church, and are for use by that Church are acceptable as per the Rule of the Order. Honorary Degrees must be listed as Honorary.

If an application contains questionable Orders, titles, or degrees it is at the discretion of the President of the Association, to not have them included on the application. If this is the case, the applicant will be notified, the decision is final, and the ASSOCIATION will ask the applicant not to use these Orders, titles, and degrees within the ASSOCIATION. Such action, if deemed appropriate, can occur during any stage of membership. This includes theapplication process of any position or Title of Honor. Any grievances will be handled according to the Rule of the Association.

All applications must be accompanied by a Curriculum Vitae (CV), bio, or equivalent. The application must be signed and the applicant will be notified of the proper address to submit the completed application. An original application will be on file with the Grand Registrar and copies will be maintained by the Association.
