I wish to inform the Graduate School that I have chosen to pursue the Ph.D. degree in the University Program in Genetics and Genomics and with ______as faculty advisor.
I therefore request to affiliate with the University Program in Genetics. The Director of Graduate Studies of the Program and my thesis advisor has given their approval to this affiliation by signing below.
Affiliation with a lab represents my commitment to engage in and complete a research thesis that will partially fulfill the requirements for a PhD. I understand I am expected to work regularly in the lab (and/or field), maintain consistent contact with my mentor, interact with the rest of the lab, and continue to participate in UPGG activities.

In accepting ______(student) as a thesis student in ______(mentor's) lab:
***UPGG does not backstop students financially once they affiliate with a UPGG faculty lab. Accepting a student into one’s lab means that the PI, and/or his/her Department, is accepting financial responsibility for the student.***
The mentor is responsible for the financial support of this student through the completion of the Ph.D., at the annually defined salary level determined by the UPGG Program and the Graduate School. However, if the advisor fails to provide this support it will be provided by the ______Department. If the student receives another award or assistantship and the stipend level is less than the established stipend, or the educational allowance is less than the amount required for fees and health insurance, I will make up the difference from departmental sources. Financial support will begin at the start of the student's third year and continue for at least three calendar years. A letter regarding exact date will be sent to the mentor and departmental business manager at the end of the student’s second year.

I, as mentor, agree to allow the student to complete the requirements for a certificate in Genetics, which include taking the prelim examination during the fourth or fifth semesters, completing a research thesis, publishing one peer-reviewed manuscript by the time of the thesis defense, as well as attendance at UPGG meetings and seminars.
I realize annual committee meetings (beginning after prelims are passed) are required, and written reports of the meetings should be sent to the UPGG office.
