Submitted Questions

Q1. Is the supplier billed directly for the bulk CO2 or is the University billed and you submit for reimbursement?

A1. The CO2 supplier bills directly to the beverage vendor.

Q2. What is the estimated annual Juice volume?

A2. We purchase a juice concentrate. The dilution rate to water varies from 4:1 or 5:1 and produces 36 or 45 liters of finished product. From 5/1/15 – 4/30/16 the University purchased 1,752 cases (3 –3Liters containers/case or 9 liters).

Q3. How many juice machines do you currently have in all locations and what price are you paying for it?

A3. There are a total of 6 dispensers on campus. There are 4 in Watterson and 2 in Linkins Center. Each machine dispenses 4 products.

The pricing per case varies from $57.10/case - $79.81/cs. The average price per case based on sales and usage is $71.58.

Q4. Section 5.2 Supplies/Services Required 4a. University Background Checks- Excluding having our employees fingerprints ran that will be on campuswhat else is

involved in the background check process?

4b. What is the process of completing the background checks?

A4. The link associated with the RFP at the end of section 5.2 is not active. The documents attached with this addendum provide guidance,but do not need to be submitted with your proposal.

Q5. State of Illinois Requirements Section: Are vendors currently delivering to the ISU campus required to register on the Illinois Procurement Gateway Website?

A5. Vendors are not required, but are strongly encouraged, to register on the Illinois Procurement Gateway (IPG) Website. By registering on the IPG, vendors can submit required information (disclosures, registrations, etc.) in advance electronically instead of on paper. A vendor is only required to submit the full amount of information once per year. All state agencies and institutions have access to the vendor information on the IPG. This means that vendors are not required to complete separate documents for each solicitation.

The fingerprintingmustbecompleted andresultsevaluated by the University priortothe begin date ofyouremployment/ work on Laboratory school property.

Human Resources has arranged for IdentoGO (formerly L-1 Enrollment Services Division) to fingerprint ISU employees, applicants, and vendor employees. Their offices are located at 1701 Empire Suite 360 in Bloomington and at Crossroads Center, 482 Wylie Drive in Normal. They are available for fingerprinting Monday through Friday. You may call 1-800-377-2080 immediately to schedule your fingerprinting appointment. Tell IdentoGO it is for Illinois State University and the billing account number is L12817S. Or you may schedule an appointment online at by following the procedures listed on the on the next page.

Upon arrival at your appointment, you must show either your driver’s license or another valid State or Federal photo ID.

Fingerprint results will be sent directly to Human Resources for review pursuant to University Policy 3.1.31 Criminal Background Investigation Policy and/or any contract or agreement with the University.

Background Check Terms

Illinois State University and the Vendor are entering into a contract/agreement the (“Agreement”) and these Terms and Conditions are incorporated into the Agreement. In the event either a Standard Background Check or a Lab School Background Check is required by law or the University determines one is necessary, the Vendor agrees to abide by the terms and conditions set forth below.

General Requirements:

  1. The University shall notify the Vendor in the event a Standard Background Check or a Lab School Background Check is required or necessary. Whether a background check is required or necessary is in the sole discretion of the University.
  2. These Terms apply to any subcontractor used on the Agreement and Vendor is required to include these Terms in any subcontract used in the performance of the contract.
  3. When either a Standard Background Check or a Lab School Background Check is required, the Vendor shall:
  4. Complete and/or facilitate completion of the required background check.
  5. Comply with all identification badge procedures required by the University for Vendor employees subject to background check.
  6. Assure sufficient numbers of employees (or employees of subcontractors) have successfully completed the applicable background check such that the Vendor can timely complete all work as specified in the Agreement.
  7. Not permit employees (or employees of subcontractors) to substitute or amend work schedules such that work is performed by individuals who have NOT successfully completed the required background check.
  8. Perform, as necessary, the applicable background check on all Vendor employees working on this Agreement, and for any future Vendor employees or any subcontractor employees working on the Agreement.
  9. Retain all Standard Background Check information performed as outlined above, and provide that information to Illinois State University upon request.
  10. Agree to maintain information concerning the results of a Laboratory School Background Check as confidential information that may not be disclosed unless authorized by the individual who is the subject of the background check or as otherwise required by law.
  11. Timely notify Illinois State University regarding addition, deletion, or other change in status of any employee for whom a background check is required.
  12. Report any change in the criminal history conviction status of any Vendor employees (or subcontractor employees) to Illinois State University Purchasing Office 100 S. Fell, Suite 215, Campus Box 1220, Normal,IL 61790-1220.
  13. The University shall not be responsible for damages to the Vendor for delays, or costs increased/incurred due to the Vendor’s failure to ensure completion of a required background check or sufficient number of individuals who have successfully completed the required check. If such delays are the result of causes within Vendor’s reasonable control or power to avoid, Vendor will not be entitled to any time extension for completion of work.
  14. Utilizing the University badge process, the University shall conduct regular checks to ensure that employees have completed the applicable background check requirement. In the event that employees are working who have not completed the required check, the University reserves the right to suspend work – at no penalty to the University – until the Vendor has completed the required checks and or substituted appropriately cleared personnel.
  15. The University reserves the right to immediately terminate the Agreement without penalty to University for any violation of the Background Check Terms by the Vendor.

Standard Background Check: (NOT Applicable to Lab School projects)

  1. Within 15 days of notification the Agreement requires a Standard Background Check, Vendor shall complete the Standard Background Check for all Vendor employees working on the Agreement, and upon completion of the required checks, complete and submit Form A: Standard Background Check Affidavit together with a list of all Vendor employees for whom it has completed the Standard Background Check prior to any work commencing under the Agreement. These materials shall be submitted to:

  1. Vendor acknowledges no work can commence under the Agreement until all required Standard Background Checks for Vendor employees (or Subcontractor employees) are completed.
  2. A Standard Background Check includes the following checks for each Vendor employee:
  3. A search of a national criminal database, or accessing multiple state repositories simultaneously of states where the individual being checked has resided, and that includes, but is not limited to at least 7 years of information, including felony, misdemeanor and other infraction information.
  4. A name check of the national Sexual Offender Registry and a check of the Illinois Sex Offender Registry , and
  5. A check of the Murderer and Violent Offender Against Youth Registry.
  6. Vendor agrees to comply with the requirements of all applicable laws, including the Fair Credit Reporting Act, when completing a Standard Background Check including but not limited to:
  7. Notification to employees and receipt of permission to get a background report;
  8. Not discriminating against the applicant or employee, or otherwise misuse the information in violation of federal or state equal opportunity laws or regulations.
  9. Vendor acknowledges that it is responsible for determining if an employee has successfully completed a Standard Background Check and responsible for clearing employees for work at the University and also agrees that Illinois State University has the right to ask for the removal of any Vendor employee or subcontractor employee.

Lab School Background Check

  1. A Lab School Background Check is conducted by the University through the Human Resources Department. A Lab School Background Check includes:
  2. A search of state and Federal Bureau of Investigation fingerprint databases.
  3. A name check of the national Sexual Offender Registry and a check of the Illinois Sex Offender Registry ; and
  4. A search of the Illinois Department of Children and Family Services Child Abuse and Neglect Tracking System.

Each criminal history records check shall be valid for one year from the date on which it was completed.

  1. Lab School Background Checks are required for any Agreements that involve services or work in or around the University Laboratory Schools (shall include the premises of the Thomas Metcalf Laboratory School, University High Schools, Rachel Cooper or Fairchild Hall, the adjacent areas, or any parking lots used for Laboratory School purposes), or other areas designated by the University in its sole discretion.
  2. Within 7 days (or as otherwise mutually agreed by the parties in writing) of notification the Agreement requires a Lab School Background Check, Vendor shall provide University with completed Form B, which includes a List of all Vendor employees (and subcontractor employees) who will be working under the Agreement.
  3. Within the time period designated by the University, Vendor shall ensure that all Vendor employees identified on Form B will complete and submit the Form B Authorization Form permitting the University to complete a Lab School Background Check by submitting the Authorization Form to Illinois State University Human Resources at:

  1. Vendor acknowledges that no Vendor employee (or employee of subcontractor) may perform any work at the University Laboratory Schools until the required Lab School Background Check is successfully completed, in the University’s sole determination.
