Job Description
Job Title:Community Connector – Children & Families
Hours of Work:25 hours per week to include evening and weekend work, as required
Responsible to:PAVS 3rd Sector Health & Well-being Facilitator
Location:36/38 High Street,Haverfordwest
Salary:NJC scale points 21-25 (£20,138 - £22,658 pro rata)
Holiday entitlement:25 days per annum+ Bank holidays(all pro rata)
Duration of contract:Initially to 31st March 2018
Aim of the post
To facilitate the involvement of 3rdsector organisationsin the delivery of services, activities and volunteering opportunities for families that access support from the Families FirstTeam Around the Family (TAF) programme
Duties and Responsibilities
- Liaise with the Team around the Family (TAF) andthe Family Information Service (FIS) to embed systems to broker 3rd sector services into packages of support for families
- Support families referred to TAF to access services, activities, volunteering and participation opportunities offered by volunteer-led community-based third sector groups
- Liaise with the locality-based Community Connectors and the Time Banking Development Officer to ensure that children and families are offered the widest range of opportunities to improve their well-being
- Support 3rd sector providers of services to children, young people and families to upload information about their services onto infoengine (a regional on-line directory of 3rd sector services)
- Work closely with the TAF Manager and the Families First Support Team and provide regular reports on activity and outcomes to the Steering Group, as required
- Broker third sector services and voluntary activities into other partnerships that support children and young people, as required
- Support the network of third sector groups in Pembrokeshire that work with children and young people in order to disseminate information, share learning and best practice, and engage with service users
- Attend meetings of the Pembrokeshire Play Strategy Partnership and other partnerships that may be identified as being useful in terms of developing third sector services for children and families
- Ensure that third sector groups working with children and families are signposted to appropriate support in respect of governance, safeguarding, quality assurance, volunteer management, etc
- Attend appropriate courses, workshops, seminars and conferences to meet identified professional development needs
- Undertake any other duties as may be reasonably required by PAVS Chief Officer, theChildren & Families Executive Group, or the Families First Steering Group.
Community Connector – Person Specification
Qualifications, knowledge, experience / Essential / DesirableGood standard of education (degree level or equivalent) or ability to demonstrate relevant experience
In-depth knowledge and understanding of the third sector in Pembrokeshire
A broad understanding of the statutory health and social care provision within the County
Experience of working with a wide range of organisations, including statutory partners and third sector groups
A good understanding of asset-based approaches to community development and individual well-being
Current full driving licence and access to a vehicle for work /

Able to work on own initiative as an “agile worker” and to organise/prioritise work as necessary to meet deadlines
Excellent communication, facilitation and participatory engagement skills
Ability to present information in a range of formats including written reports, verbal presentations, graphs, charts, etc
Competent user of ICT applications and on-line communication tools and confident enough to use wireless technology in a range of community settings
