Section: Occurrence Management / Subject Title: Occurrence Classification Procedure

Purpose:

To classify occurrences according to the type and severity.

Responsibility:

Quality Improvement Committee members shall assign occurrence classification upon reviewing all incident reports at QA meetings.

Procedure:

Occurrences shall be classified in 2 parts:

  1. Severity of the occurrence

I / Severe / Adverse outcome without successful intervention.
II / High / Adverse outcome with successful intervention.
III / Medium / Minor self-limiting adverse outcome.
IV / Low / No known adverse outcome.
