SUBJECT: Revised Wage Information Request (Form CS-130) and Procedural Instructions

The CS-130 and its procedural instructions have been revised to update the format and to reflect technical changes to be consistent with other forms and procedures. The CS-130 and their procedural instructions have been updated on the CHFS Intranet Forms page. The Spanish version of the CS-130 will be added to the Intranet once it has been translated by the Cabinet for Health and Family Services Language Access Section.

The following revisions have also been made to the CS-130 and its procedural instructions:

·  Changed “Cabinet for Families and Children” to “Cabinet for Health and Family Services” due to reorganization of state government.

·  Replaced the “Children First” with the “Unbridled Spirit” logo per CHFS Secretary’s instructions.

·  Changed the term “noncustodial parent or obligor” to “noncustodial parent” per Child Support Memorandum No. 30.

·  The instruction to return a business reply envelope was removed as contracting officials do not have business reply envelopes and the procedural instructions were revised to allow the CS-130 to be sent to the employer by mail or fax based on comments.

·  A line for fax number was added under CHFS address based on comments.

·  The CS-130 was revised to remove the question on where to send the income withholding order as it should be sent to the new employer if known.

This transmittal letter is to be saved and filed for future reference as it explains changes that have been made to the Kentucky Child Support Forms. The Child Support Forms are to be updated as shown below and on page two.
