The education approach discussed in this example can be used in any health care settings across the continuum of care. Regardless of the setting, methods of instructionto develop instructors who can integrate clinical expertise with technology and hence be our leaders of clinical transformation should focus on critical thinking and the use of information. The instructional methods used in the Instructor Development Workshop have been used in hospitals ranging in size from 30 bed facilities to 1000+ bed facilities, single purpose institutions and multi-site systems. Some hospitals haveused these methodologies for education with ambulatory clinics, SNF, and acute rehab facilities.

AbingtonMemorialHospital is a fully accredited, not-for-profit, 570-bed, regional teaching hospital in Abington, Pennsylvania. AMH has been providing comprehensive, high-quality services for people in Montgomery, Bucks and Philadelphia counties for over 90 years. With more than 36,000 inpatient admissions annually, AMH is a major regional referral center for cancer care, cardiac care, and surgery (including orthopaedic surgery and neurosurgery) and has the only level two trauma center in MontgomeryCounty. Over 86,000 patients come to the EmergencyTraumaCenter each year. AMH has a long tradition of personal, high-quality maternity care and is also a leader in senior health services.

AbingtonMemorialHospital has a strong educational mission and sponsors five residency programs - in family medicine, internal medicine, obstetrics/gynecology, general surgery and dentistry. In addition, the hospital provides postgraduate medical education in affiliation with several area medical schools. The hospital also operates theAbington Memorial Hospital Dixon School of Nursing, and has programs in radiologic technology, nuclear medicine and medical technology.

The Instructor Development Workshop targets all clinicians: RNs, RT, RD, RpH, PT, OT, SLP, MSW, SW, Chaplain, Case Management, Child Life, Music Therapy. Site leaders are encouraged to ask providers with excellent clinical expertise to be instructors and super users. Individuals who are clinical leaders in their practice and very patient focused in their care delivery are ideal candidates.

  1. Summary of the instructional approach

The Instructor Development Workshop is an approach to developing Instructors and competence with a clinical application that is not the typical “train the trainer” class. This workshop is intended to engage the participants in clinical discussions, tap their clinical expertise, and help them leverage the functionality in the computer program to enhance communication among providers on the interdisciplinary team, document to full scope of their professional accountabilities, and tap the potential of all end users to do the same. This workshop supports professional processes of care and focuses on the use of clinical knowledge by incorporating the use of evidenced based guidelines and a professional practice framework as a foundation.

The Instructor Development workshop is a 2 – 2.5 day interactive class that utilizes a variety of learning strategies, e.g.

  • Small group discussion,
  • Large group discussion
  • Demonstration
  • Hands on computer time
  • Clinical context Scenario Exercises /application
  • Games
  • Individual reflection,
  • Individual and group analysis and application of principles
  • Short stations with focused concepts for exploration

Day one is to help instructors gain basic awareness and familiarity of the application within the clinical context of a professional practice framework.

This day focus on the “benefits” for the clinician, the patient/family and the organization.

Day 2 – 2.5 is to deepen the comprehension and utilization of the professional practice framework, evidenced based clinical practice guidelines, clarity on scope of practice and the potential this has to impact patient care. Interactive targeted discussions are conducted within clinical context. These days focus on comprehension, workflow, confidence and being able to communicate and teach others. AbingtonMemorialHospital offered 3 additional days to their instructors to further enhance these skills.

  1. Interdisciplinary Discussion Promoting Increased Understanding:

Understanding of each disciplines scope of practice and their unique contributions to care were articulated. As disciplines shared their thinking about patient care, each discipline expressed awe at what one another do. There was enhanced mutual respect for others.

  1. A deeper understanding of how to use the computer was enhanced by having a variety of learning strategies.
  1. Use of Analysis in the Instructor workstations helped with comprehension and retention for instructors.

  1. More dedicated time for instructors to internalize the benefits of embracing and living a professional practice framework in the provision of care.
  2. Create follow up time to pull instructors back together after they have had time back on unit to reflect and practice on the computer in their setting. This meeting/class could provide additional information so that instructorsrepeat information from their “lived experience” so they can effectively pass on to others the benefits of the new system. Create own clinical examples.
  3. Sites should take time to personalize the presentation and use examples that are clinically relevant for their organization.
