/ Template: register of people with significant control (academies)

This template register of people with significant control (PSC) is split into two parts, and which one you complete will depend on whether your academy has any identifiable PSC.

It is based on guidance from the Department for Business, Innovation and Skills (BIS).Annex 2 of the guidance provides the official wording that companies must use to complete the register, and information about what information must be recorded in different situations.

Use this section if your company has no PSCs or is still in the process of identifying its PSCs.

No identifiable persons / Date the statement ceased to be true
This section should be completed using one of the following statements:
  1. The company knows or has reasonable cause to believe that there is no registrable person or registrable relevant legal entity in relation to the company
  2. The company knows or has reasonable cause to believe that there is a registrable person in relation to the company but it has not identified the registrable person
  3. The company has identified a registrable person in relation to the company but all of the required particulars of that person have not been confirmed
  4. The company has not yet completed taking reasonable steps to find out if there is anyone who is a registrable person or a registrable relevant legal entity in relation to the company
Where any of the above statements are no longer applicable, you must enter that on your PSC register along with the date on which the statement ceased to be true.
If statement 1 applies to your academy, you will not need to fill in any more details on the register.
Notices issued / Date of issue
This section should be completed to record details of notices you have issued in order to identify your PSCs. More information on issuing notices can be found on pages 39-44 of the BIS guidance, linked to above.
The official wording to use to record the issuing of notices can be found on page 50.

Use this section if you are able to confirm details of your PSCs. You can copy and paste this table multiple times if you have more than one person with significant control to record.

Name / Date of birth / Nationality / Country, state or part of the UK where the individual usually lives / Service address / Usual residential address, if different to service address
(This must not be disclosed when making your register available for inspection or providing copies of the PSC register) / Date of becoming a person with significant control / Has an application been made for the individual’s information to be protected from public disclosure?
Which conditions for being a person with significant control are met / Date he or she was added to the register
