This is the checklist that I put on the cover of the folder.
In the pale yellow column, I type the name of the person who should be given that form before or after the meeting, because various paperwork may need to be provided to various paperwork at different points in time (Campus coordinator, LSSP, etc.) I write the date that I put it in their box over their name with a vis-à-vis.
In the pale green area, I document with a check/date when forms are complete and ready for the meeting. If an area has two places to check, it’s because there are two steps to the form that I need to document (for example, that I sent the notice home and that it was returned to me signed).
All of the notices, surveys and other loose leaf forms that I receive leading up to the meeting go inside the folder. I also preprint certain pages that I put in the folder to save time during the meeting, such as SHARS, signature page and prior written notice.
Because the folders are laminated, I am able to document these dates with a vis-à-vis leading up the meeting, but can erase the folders and reuse them once the meeting is over.
Documenting these dates is useful for the special education teacher because we can always provide information regarding whether or not and when we received paperwork and how many times we’ve attempted to accomplish a task and with whom.
I’ve put question marks over page/screen numbers for you on all documents I am sending because this information various by district.
On the inside of the folder I glue in the “script.” I can even check off parts of the script as I go down to remember what I have discussed and what has not yet been discussed. Again, these markings can be erased later.
On the back of the folder I glue a copy of the Hierarchy of Cuing and Prompting to be used as a reference when speaking with parents.
