Project Description Form (Projects)

This template should be used as a prompt to assist the project development discussion(s) with an organisation. Following this meeting the organisation must complete what is called the ‘Project Description Form’ onlinewhile logged in to the LV website before a project can begin.

New Project Submission Processes for Member Organisations

  1. In collaboration with your GC Client Manager and using Word versions of the Project Description Forms as prompts/drafting tools, scope projects to a level of detail Client Managers find appropriate.
  2. Organisation representative to submit the final version of the appropriate GreatConnections Project Description Form online: Access the LV website using the login details provided at sign up, go to the menu for ‘GreatConnections’, select ‘Project Description Form’, complete the form.
  3. GC Team will notify Client Managers when the Form has been submitted.
  4. Client Managers will begin matching or tweak the project description with you further as needed.

ER Victoria hosts a quarterly member’s network. The meetings provide a forum for agency representatives to come together to share information, discuss current and emerging trends, discuss best practice, hear from guest speakers and meet with government departments and other stakeholders who are interested in seeking feedback from the Victorian emergency relief sector.

THE PROJECT

Title of the Project:“Are we better off? An evaluation of the impact of peak ER Victoria on the sector”
Description of the Project /The Need
ER Victoria (ER Vic) is the peak body for the state emergency relief sector. It aims to bring emergency relief agencies together, to support their work, and to advocate on issues affecting vulnerable and disadvantaged people. This project would provide a brief evaluation of the impact the organisation has had on the sector.
While the ER Victoria network has operated for over 30 years, as a newly incorporated organisation, it received funding from the Federal Department of Social Services (formerly Families, Housing, Community Services & Indigenous Affairs) in 2011. This funding enabled ER Vic to employ a full-time Sector Development Manager and officially commence operation as a peak body. Over the course of the last 3 years, ER Vic has undertaken a range of activities to implement its strategic plan. This evaluation will be used to demonstrate the importance of a funded sector development position and to assist in identifying key needs and opportunities to strengthen and support the sector into the future. This report will be used to help attract philanthropic funding in order to continue the work of ER Victoria.
We anticipate that the evaluator would engage with key stakeholders including but not limited to members, staff, volunteers, committee of management, peer bodies, philanthropy and government departments.
Preferably, a short written report would be produced.
Time frame – commencement asap with finish date by 28/02/15
We are seeking a skilled and experienced evaluator, with strong interpersonal and report writing skills.

By completing the online project description form, the organisation agrees that this is the project agreed between them and the GreatConnections Client Manager.

As Chief Executive Officer (or equivalent) of the organisation, I certify that the information given in this application is true and correct. All appropriate information about our organisation will be made available to the skilled volunteer provided by GreatConnections. I agree to liaise with GreatConnections volunteers with courtesy and respect; ensuring that when contacted by a Client Manager, Volunteer or member of the GreatConnections Team,I will return the phone or email within five working days.I agree to report to GreatConnections on the progress and outcomes of the project/activity when requested to do so.

I further note that my organisation accepts that the people participating in this GreatConnections placement do so as volunteers of my organisation and neither as representatives of their own employer nor as agents of Leadership Victoria. Where critical matters are involved I acknowledge that my organisation will seek independent professional advice. Any person assigned to participate in the project from GreatConnections will be treated as a volunteer with my organisation (s) for insurance purposes. I accept responsibility for hosting volunteers within my organisation and have in place volunteer and/or employee policies that meet with the National Standards for Involving Volunteers in Not-for-Profit Organisations.

Leadership Victoria does not guarantee to successfully match every request from a non-profit organisation through GreatConnections. Whilst we endeavour to make the best possible match for each organisation and supply a candidate who is appropriate to the organisation’s needs, it is the organisation’s responsibility to assess volunteers and satisfy themselves as to the suitability of the selected person.

Leadership Victoria does not assess the suitability of any volunteer for any position within your organisation or verify or take responsibility for the accuracy of a candidate’s application and is not responsible for any issues that may arise in respect of the arrangement between the candidate volunteer and the organisation, including any incorrect details which may be contained in a candidate’s application or any misrepresentation which may be made by a person. Leadership Victoria recommends that organisations undertake careful due diligence through referee checks etc.It is the sole responsibility of the Organisation to carry out the appropriate Police and Working with Children Checks in relation to any Great Connections Volunteer. Leadership Victoria takes no responsibility for the conduct of these Checks.

Each organisation must take full responsibility for its own decisions in selecting and appointing candidates. Leadership Victoria has no liability, in negligence or otherwise, for the ultimate suitability of candidates or for any acts or omissions on the part of individuals appointed through GreatConnections.
