This document is a brief description of agency benefits for full time employees.

Some benefits are available for employees working part-time.

Effective Date of Insurance Coverage is the 1st day of the month following 30 days of employment

HEALTH INSURANCE

High Deductible Health Plan (HDHP) with Health Savings Account (HSA) / WorkAbilities health insurance is provided through HealthPartners.Coverage consists of a high-deductible insurance plan with an accompanying health savings account (HDHP w/HSA). Family coverage is available, and is the financial responsibility of the employee.
WorkAbilities pays $357.82/month of the plan premium and contributes $143 to the employee’s HSA. The employee is responsible for $187.32/month. The annual deductible is $3500.

DENTAL INSURANCE

/ WorkAbilities dental insurance is provided through Delta Dental of MN.
WorkAbilities pays 100% of the monthly premium for the employee. Family coverage is available and is the financial responsibility of the employee.

LIFE INSURANCE

/ WorkAbilities life insurance is provided through Unum Insurance Co. It provides employees $10,000 benefit term life insurance atno cost to the employee.
LONG TERM DISABILITY
SHORT TERM DISABILITY*** / WorkAbilities provides long term disability insurance through Unum Insurance Co. at no cost to the employee.
***Employees may alsopurchase short term disability coverage at their own expense from Colonial Life Insurance Co.

403 B RETIREMENT PLAN

/ WorkAbilities offers its employees the option of participating in a 403B tax-deferred annuity retirement program. WorkAbilities may make a discretionary matching contribution to the plan at the end of the year.
FLEX BENEFIT PLAN
(FLEX SAVINGS ACCOUNT) / WorkAbilities offers its employees the option of participating in a Section 125 Flex Benefit Plan (Flex Spending Account). This plan allows participating employees to pay health care and dependent care expenses with pre-tax dollars, thus making them more affordable. Employees can also pay their group insurance premiums through this plan, with pre-tax dollars.

PAID TIME OFF

/ WorkAbilities’ employees earn the following:
Years 1-2 15 days of paid time off
Years 3 – 10 18 days of paid time off
Years 11 – 20 21 days of paid time off
Years 21 + 24 days of paid time off

HOLIDAY

/ WorkAbilities provides 8 paid holidays per year:
New Year’s Day Martin Luther King, Jr. Day Memorial Day July 4th Labor Day Thanksgiving & day after New Year’s Eve

PAY DAYS

/ Employees are paid every two weeks on Friday. Employees may choose either paper checks or direct deposit.

TUITION REIMBURSEMENT

/ WorkAbilities, Inc. will reimburse full time employees 50% of tuition for one approved and successfully completed college or continuing education course per semester.

OTHER BENEFITS

/ Paid lunch break of 45 minutes, free parking, free Notary Public services, smoke-free work environment.
