Form 2: Residential Development Experience
¨Answer “All” # Residential Experience Questions (Word/ PDF)
¨Fill out Residential Experience Development Chart in assigned order (Excel)
(Other excel spreadsheets will not be accepted).

Form 3: Residential Management Experience
¨ Only Managing Agent must answer “All” # Management Questions (Word/ PDF)
¨Fill out Management Experience Development Chart in order (Excel)
(Other excel spreadsheets will not be accepted).

Form 4: Active Pipeline
¨ Capacity: Active Pipeline excel chart
Form 5: Principal Property Listing
¨ Property Listing Chart (Excel)
(Applicant principals must disclose DEP liens on the property listing).

Form 6: Assets Statement & Compiled Financials Statements Prepared by CPA
¨ Compiled Financial Statements: cash flow, balance sheet, and income statement; or
¨ Reviewed Financial Statements: cash flow, balance sheet, and income statement; or
¨ Audited Financial Statements: cash flow, balance sheet, and income statement; and
¨ YTD operating statement
¨ If applicable, For-Profit Applicants shall also submit a completed Assets Statement, Form 6, for at least one principal.

Form 7: Bank Reference Letter
¨ Bank Reference Letters from all the banks that the applicant has the banking relationship.

The reference letter shall mention the total outstanding loan size, payment history (delinquency history, if any), and the Bank officer’s contact information. The reference letter needs to be signed by either the Lending Officer or the Servicing Manager.

Form 8: Application Letter
¨ Application Letter to Assistant Commissioner (Dated and Signed).

Required:
¨USB Flash drive with RFQ application (PDF) and Excel Charts

¨ $100 Application Fee “NYC Department of Housing Preservation and Development.” (For-Profit)

¨ $100 Application Fee “NYC Department of Housing Preservation and Development.” (Not –For-Profit)
(Certified Check, Cashier’s Check or Money Order, nonpaying fee applications will not be reviewed).
