The first attachment above is your certificate of insurance and Automatic Additionally Insured document. This document provides automatic additional insured statusfor the venue where you perform and/or your client.

FormGMI GL 18 0606, attached, eliminates the need for you to request additional insured certificates. If you are asked to add an additional insured, forward this whole email to that person. In the description section of your certificate we include the additional insured form GMI GL 18 0606 so vendors and clients can confirm they have the additional insured coverage they desire without further processing.

If you purchased scheduled equipment insurance,your scheduled equipment list must be in our office prior to a loss for that equipment claim to be covered. Complete and return the attached excel spreadsheet. If you haven’t purchased the equipment coverage, complete and return it for a quote.

Below is an introduction letter for venue contacts. You should include this letter when providing proof of insurance.

In June 2006 the insurance industry created a form that expedites Additional Insured requests by including that coverage for venues in the policy. Willis & the National Association of Mobile Entertainers (NAME) are the first to use form GMI GL 18 0606 to eliminate unnecessary processing.

Please verify your venue contacts have read the Automatic Additionally Insured Status Certificate, attached. The venue contact will read coverage is provided under Section II #2 of the form. Every venue that receives a certificate issued from Willis for every NAME member automatically has additional insured status without the need for further follow up. This form is an industry standard across theUnited States. Every risk management or legal department of every entity that has reviewed this form has accepted it including the Marriot, Hyatt, Ritz Carlton, Trump,Anheuser-Busch Companies(Busch Gardens & SeaWorld), Walt Disney World, etc. All privately owned catering halls and restaurant’s insurance brokers have approved it as well.

NAME Member --

This program is green and has been designed to be user friendly by eliminating the need for additional certificate processing. This Automatic Additional Insured Status form and your certificate should be emailed to any entity that requests evidence of insurance or to be named as an additional insured.

This program utilizes form GMI GL 180606 that was approved by the Insurance Departmentin June of 2006. A few entity contacts have still not seen this before and only look for their name on a COI. If they read Section II #2 of the form they will understandthat the coverage theyrequirehas already been included for their convenience. If they do not have authority to approve this version of additional insured status request they forward it internally to the person that has that authority or their risk management or legal department.
