The Transfer Review (conversion) Process

SEN Officers must contact schools at the beginning of each half term to identify the dates of the transfer review meetings. These dates must be entered onto the tracking spreadsheet in order to monitor the 20 week timescale.

Minimum of 1 week prior to Notification letter being sent / Support Officer merges EHCP to prepopulate basic information
EHCP Coordinator has Family Conversation Meeting to discuss history, aspirations etc. to formulate Section A using person centred planning tools. Also ascertain if new advice is being requested.

EHCP Coordinator to send prepopulated plan, agenda etc. to all attendees at least 1 week prior to Transfer Review Meeting.
If professionals unable to attend seek their views for consideration at Transfer Review Meeting.

Proposed plan discussed and co-production of draft EHCP with a focus on outcomes, school requested (if appropriate)

Check and prepare co-produced plan
EHC Plan Checking
Team Manager to ensure EHC Plan checked against Quality Assurance Documents and amendments made to plan if necessary

12 / ISSUE DRAFT EHC PLAN / PROPOSAL TO CEASE TO MAINTAIN STATEMENT/ FEEDBACK FOLLOWING AN EDUCATION, HEALTH AND CARE TRANSFER REVIEW
13 / Mainstream schools consulted(special school if originally in this type of placement)
Parents/Young person able to respond to Draft EHC Plan with request for amendments/school place/change in provision

16 / Case taken to Provision Panel if specialist provision requested and specialist provision consulted if agreed
If concerns raised by a school in relation to consultation these will be responded to appropriately in writing
Make appropriate amendments to plan following parental requests/discussion
If there are particular difficulties arrange a meeting with the parents/young person. / FINALISE CEASE TO MAINTAIN STATEMENT

NB The DfE Statutory Transition Guidance September 2015 states:

"The local authority must not seek any advice required for an EHC needs assessment if such advice has been previously provided for any purpose and the person providing that advice, the local authority and the child's parents or the young person are satisfied that it is sufficient for the purposes of an EHC needs assessment" 5.10 p23
