The PTA/PTSA scholarships are one-time cash awards, payable directly to a graduating senior in the Berea City School District planning on attending an accredited school, college or university in the Fall of 2016. PTA/PTSA scholarships are awarded at the Senior Awards Night.

THIS APPLICATION MUST BE COMPLETED AS A TYPED DOCUMENT. NO APPLICATIONS OR ESSAYS THAT ARE HAND-WRITTEN WILL BE ACCEPTED.

UPON COMPLETION OF YOUR APPLICATION, PLEASE STAPLE YOUR ESSAY TO THE BACK OF YOUR APPLICATION.

Criteria for Essay:

Length – 1 page, typed, double spaced

Font – Times New Roman, size 12

ESSAY TOPIC: DESCRIBE A CHALLENGE THAT YOU HAVE FACED OR OVERCOME.

Please remember that only your Student ID # must be on your essay.

Only one application is needed to apply for all available scholarships presented by any of the PTA/PTSA units in our school district. Each PTA/PTSA units, as well as, Berea City School District PTA Council, have scholarship committees established to select the PTA/PTSA scholarship winners.

In order to be fair to all students, the scholarship review committees will not know who you are. Please do NOT mention your name on any of your remaining application answers or typed essay.

The scholarship application MUST be completed in its entirety. Incomplete applications will NOT be considered for an award.

The deadline for application submission is Thursday, March 17, 2016. Completed applications are to be returned to your guidance office before the end of the school day.
