The Phoenix Medical Practice is asmall, friendly, well established,single-handed,undergraduate teaching practice. DrKhan is the principle GP, and your named accountable GP.The Practice is proud to offer the highest standard of patient centred healthcare. We run many clinics for the management of chronic disease (eg, asthma, CHD, diabetes) and offer a wide variety of other medical services including minor surgery, childhood vaccinations and well person checks.

We are a member of the Family Doctor Association (FDA). The Family Doctor Association leads the way to the future for grassroots primary care. With over 1000 members inspired for over 30 years. Continuity of care benefits patients, improves health outcomes and saves the NHS money. Disabled Access

To enable access to the surgery for disabled patients we have the facility of a ramp and double doors for easy access. The practice is based on the ground floor level only.

Patient Group (PPG)

The practice has a successful virtual patient representation group so that you can have your say. We will ask members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time. If you are happy for us to contact you occasionally by email, please ask the receptionist for a sign up form or go onto our website and complete the sign up form. We will be in touch after we receive your form. Please note that no medical information or questions will be responded to. We welcome all patients of the practice to join.

A range of nurse and doctor morning and afternoon appointments are available. If you wish to see a particular practitioner, please inform the nurse/receptionist when booking an appointment.

T.A.R.G.E.T. Staff Training

The practice is closed on some Wednesday afternoonsfor T.A.R.G.E.T. Training from 12.00pm. This enables all staff (doctors, nurses and administration) to keep fully up to date with modern medical practice in Primary Healthcare.

When We Are Closed

If you need urgent medical advice or assistance when the practice is closed, please phone the practice on 01302 323992 and you will be transferred to the Out of Hours Service.

The Out of Hours Receptionist will take your details and a duty GP will phone you back to discuss the problem in further detail. The practice receives details of this contact the following day; this enables us to update our patient medical records.

Call 999 in an emergency. Chest pains and/or shortness of breath constitutes an emergency.

Appointments

Making an Appointment &Nurse Triage

Appointments can be made by phoning the receptionist on 01302 323992. When you phone for an appointment the receptionist will need to take a few details about your problem, so the nurse can phone you back, this ensures that your problem is dealt with most appropriately. This is called Nurse Triage.

You can inform the receptionist or triage nurse if you have a preference of practitioner.

Please remember that an appointment is only for one person.

Patients who have not been reviewed for over three years can request a review appointment and consultations are available for patients aged over 75.

Patients Who Do Not Attend (DNA) for Appointments

We are a busy practice and when patients DNA, valuable doctor and nurse time is wasted resulting in less available appointments for those patients who need them. We have a policy for patients who DNA without giving prior notice, which may result in their removal from our practice list.

If you cannot keep your appointment, please cancel as soon as possible.

SMS Text Messaging Service

If you have a mobile telephone number, please inform the receptionist of this number so that we can update your records. We currently use an SMS messaging service that will send you reminders of appointments at the surgery. From 1 April 2016 this will happen automatically, so you need do nothing unless you do not wish to receive these reminders. If you do not want to receive them please see the receptionist or complete a dissent form to opt out of this service.

Home Visits

Home Visits are reserved for those patients who, for medical reasons only, are unable to attend the practice.

Home Visits should be requested before 10.30am where possible.

Visits are at the discretion of the doctor. The receptionist will need to take a few details about your illness in order to pass onto the nurse or the duty doctor who will then ring you back to gain more information about your problem and deal with it appropriately.

Telephone Advice

If you do not require an appointment but would like to speak to a Clinician, please telephone the surgery on the Practice number and the Receptionist will take the message which will be passed to the Clinician for action. Please note that any urgent medical matters should go through the nurse triage system.

Medical Students

We are an undergraduate teaching medical practice and as a result have attached medical students from Sheffield University School of Medicine who may be with Dr. Khan at the time of your appointment. If you object to this then please inform the reception staff.

Hospital Referrals

If you need a referral to the hospital we use an online booking system (e-referral) which allows you to choose your hospital or clinic and book an appointment with a specialist at a time to suit you.

CQC (Care Quality Commission)

The Phoenix Medical Practice is registered with the CQC. The CQC makes sure that hospitals, care homes, dental and GP surgeries, and all other care services in England provide people with safe, effective, compassionate and high quality care, and encourages them to make improvements. Confidentiality

All our staff have signed a Confidentiality Clause as part of their employment contract. This means we will not disclose information to your family, friends or colleagues about any medical matter at all, unless we know we have your written consent to do so.

The Protection & Use of Patient Information

There may be times when we may be required to share some of your information with other NHS staff,e.g. hospital doctors, nurses etc. This is necessary toensure you receivethe best possible health care and treatment. The law says we must pass on certain information, for example, to notify a birth. The NHS Central Register for England and Wales contains basic personal details of all patients registered with a General Practitioner (GP). The register does not contain clinical details.

Everyone working for the NHS has a legal duty to keep information about you confidential.

We are registered and comply with the Data Protection Act 1998.

Care Data

Important information about you and the care you receive is shared, in a secure system, by healthcare staff to support your treatment and care. It is important that we, The NHS, can use this information to plan and improve services for all patients. The NHS would like to link information from all the different places where you receive care, such as your GP, hospital and community service, to help them provide a full picture. This will allow the NHS to compare the care you received in one area against the care you received in another, so it can see what has worked best.

Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researchers and those planning health services, to make sure we provide the best care possible for everyone.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice reception staff.

Freedom of Information

Information about the practice required for disclosure under this act, should be made to the practice manager. (this does not include access to patients medical information)

Access to Records

In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Such request should be made through the practice manager and may be subject to an administration charge. No information will be released without patient consent, unless we are legally obliged to do so.

Comments & Complaints

We are very keen to receive your comments about the service that the practice provides to its patients. If you would like to comment on any particular aspect of our service, or if, unfortunately, you have cause to complain, please contact Mrs Paula White, Practice Manager on the practice number.

The Health Service Ombudsman

The Health Service Ombudsman has published a booklet that describes the ‘six principles for remedy’ in relation to complaints handling and involves:

Patients who are Violent and Aggressive

The Practice will not tolerate violent or abusive behaviour to staff or persons on the practice premises. Such actions will result in immediate removal from the premises and practice list. The practice will notify Patient Services immediately of any removal of a patient from the list.

Ordering of Prescriptions

We require 48 hours to process a repeat prescription

You can order prescriptions by:

  • Post or fax - please remember to tick the medicine you need on the medication list supplied with your previous repeat prescription
  • Telephone – telephone requests for repeat prescriptions are taken after 11am only
  • Online using the Doncaster PCT Repeat Prescription Service
  • In person – we have a deposit box for repeat prescription requests in the entrance to the surgery

Ordering Prescriptions Online (SystmOnline)

If you have access to the internet, it is now possible to order your repeat prescription free of charge from the practice on-line. This service will be available 24 hours a day, 365 days each year apart from short periods when the website will be having essential maintenance carried out. This website has been fully tested and has been accredited as being secure. The availability of this service is governed by the practice and is dependent upon responsible use by you, the patient. If not used responsibly, access to the online services will be revoked by the practice.

Please ask at reception to be set up with a login/password.

SystemOnline App

This is now available to patients as an app for iPhones, iPads and other Apple devices running the iOS operating system. The app is listed on the App Store and is free to download. The app is also available for android devices.

To download the app, search for “SystmOnline” in the app store. Patients can use their existing SystemOnline login details to access the online services offered by the practice via the app. The app provides a quick and easy way to change your contact details, order repeat prescriptions and view appointments. Please note that the App provides only basic functions for SystmOnline and you may need to log on via your PC for more advanced tasks.

Community Nurses

Our district nurses visit people at home when they are unable to come to the practice for treatment. They give help, advice and treatment for a wide range of conditions. Contact them directly on 01302 566999.

Health Visitors

The health visitors attached to our surgery take a special interest in the under 5’s and over 60’s. they advise on general welfare matters and availability of help etc. They can be contacted directly on 01302 737228.

Clinics

The practice offers the following clinics/services:-

Healthy Eating and Weight Management

If you require any help and advice on healthy eating or weight management please contact the surgery to speak to a Nurse. We can offer regular appointments to help you with your weight management goals

Flu Clinic

We offer annual flu vaccinations for patients in ‘at risk’ groups. If you would like a flu vaccination and fall into one of the following categories, please contact the surgery between October and December for an appointment

  • Over 65
  • Chronic Respiratory Disease including Asthma
  • Diabetes Mellitus
  • Chronic heart Disease
  • Chronic Renal Failure
  • Immunosuppression due to disease or treatment
  • Chronic Liver Disease
  • Pregnant
  • Carer for an elderly or disabled person whose welfare may be at risk if you fall ill

Respiratory Clinic

Your GP will refer you to our respiratory nurse if they feel it necessary. If you have asthma or any similar respiratory condition our nurse will wish to see you for yearly reviews. You will be sent an appointment letter when you are due for a review. The Respiratory Clinic is usually held on a Wednesday morning.

Diabetic Clinic

If you are diabetic our diabetic nurse will wish to see you either yearly or 3 or 6 monthly for reviews which include blood tests and medication reviews. You will be sent an appointment letter when you are due for a review. The Diabetic Clinic is usually held on a Monday morning.

Healthy Heart Clinic

We currently hold annual review clinics for patients with heart disease. All patients with diabetes who also have heart disease will have this check-up incorporated into their diabetic checks. You will be sent an appointment when you are due for a review, this will be a 30 minute appointment with the practice nurse, which will involve having a fasting blood test, a blood pressure check and you will be asked questions about your general health. The Healthy Heart Clinic is usually held on a Wednesday morning.

Baby Vaccination and Child Surveillance Clinic

We hold a weekly clinic every Tuesday morning for routine baby/childhood vaccinations. You will be invited to attend the practice when your child's immunisations are due. Please note that this is NOT a drop-in clinic and an appointment is necessary. You will be sent an appointment directly from the Child Health Department. When you attend for appointments please remember to bring your child’s red book with you.

The GP carries out a child health check on babies at approximately 8 weeks of age. This is usually done at the same time as the first immunisations.

Vaccinations

Routine Vaccinations for all ages are available during surgery times by appointment with our practice nurses.

Cryotherapy/Wart Clinic

We run a Cryotherapy Clinic every few weeks. Please note that this service is only available to book following referral by the doctor or practice nurse.
