SUBJECT:CTA Staff Referral Information

Here is the information about the PAR program that you requested. Please have the referral letter typed on school letterhead. The teacher’s signature on the referral is necessary, since involvement in PAR is voluntary. You will also need to print and include forms PBSD 2434 – Teacher Information Sheet and PBSD 2435 – Principal Referral Checklist when you return the packet.

The referral checklist is to be completed by a school administrator. The information on the checklist should be relevant to recent observations of the teacher by the administrator completing the checklist. Please add any comments that would assist the PAR panel in its decision. The panel would also be interested in any interventions that have been recommended by the administration.

This letter refers for inclusion into the PAR Program. The teacher has accepted the referral to PAR as indicated by his/her signature below. The teacher’s signature also provides permission to access evaluation information in the teacher’s personnel file, with the understanding that the PAR Panel and its consulting teachers will keep such information in strictest confidence.

Attached is a copy of the employee’s current schedule including the times of each period, room #, grade level, and subject taught.

Principal’s Role

  1. Referral Process – The principal or designee will:
  1. If employee is a self – referral, they will submit the referral letter.
  2. Principal will complete and return the PAR Principal Referral Checklist, PBSD 2435.
  3. Confer with the PAR consulting teacher.
  4. Review prioritized areas of concern with the PAR-consulting teacher.
  5. Receive decision from the PAR panel.
  1. Intervention – The principal or designee will:
  2. Provide support to the PAR-consulting teacher.
  3. Discuss PAR Principal Referral Checklist with PAR consulting teacher.
  4. Refrain from requesting evaluative information from the PAR-consulting teacher.
  1. Post –Intervention – The Principal or designee will:
  2. Conduct an observation of the teacher.
  3. Receive a decision from the PAR Panel regarding status of assistance.

Consulting Teacher’s Role

  1. Serves as a coach throughout the intervention/induction process.
  2. Helps in developing a variety of instructional strategies.
  3. Enhances knowledge of subject matter.
  4. Assists in planning for instruction.
  5. Creates a plan of action for the participating teacher.
  6. Orients teacher to record keeping.
  7. Orients teacher to district-wide goals and other relevant curriculum materials.
  8. Arranges for observations of other teachers.
