Overview:

The Human Resources Service Center (HRSC) and the department will facilitate the offboarding process for Faculty, Staff, GSRAs, and Temps.

Procedure

A.  When submitting departure requests to the HR Service Center
(), the department must provide the following: / Department
a.  First name / Department
b.  Last name / Department
c.  UMID number / Department
d.  Last working day / Department
e.  Reason for departure / Department
f.  Recommendation for rehire? Yes / No / Restricted / Department
g.  Letter of resignation (if applicable) / Department
B.  The department will facilitate the following for individuals who will be terminated:
a.  Keys (desk, building/office) / Department
b.  University equipment and supplies / Department
c.  University/Other Photo ID (please forward Mcard to the Mcard Center, Room 100, Student Activities Building (SAB)) / Department
c.  P-Card (please forward to Jayne Nyman) / Department
d.  MToken / Department
e.  Exchange Voice Mail:

C.  The department will facilitate the following for individuals who will transfer:
a.  Keys (desk, building/office) / Department
b.  P-Card (collect and cut up the P-Card from employees who are terminating or transferring out of your department. Notify the P-Card group about the termination with the maintenance form to close a P-Card) / Department
c.  Exchange Voice Mail:

D.  The HR Service Center will determine whether the following is needed:
a.  J-1 Scholar Notice of Departure Form (http://internationalcenter.umich.edu/immig/forms/J1_ScholarDepartForm.pdf) / HRSC
b.  Effort Certification Report (ECR) / HRSC
c.  Exit interview with the HR Service Center Supervisor / HRSC
E.  H1B-Visa:
If the employee is a foreign national on an H1B-Visa, the HR Service Center must contact the International Center at 734-764-9310. / HRSC
F.  The HR Service Center will send an email to . / HRSC
The email will include the following:
a.  First name / HRSC
b.  Last name / HRSC
c.  Uniqname / HRSC
d.  UMID number / HRSC
e.  Title / HRSC
f.  Department / HRSC
g.  Short Code(s) / HRSC
h.  Last day employed / HRSC
i.  Effective date (day after last day employed, mm/dd/yyyy) / HRSC
j.  Reason for departure (e.g., appointment completed, transferring) / HRSC
G.  Computer Access (deactivation)
The HR Service Center will notify Dental Informatics (). / HRSC & Dental Informatics
H.  The HR Service Center will note the departure date in the Time Away System. / HRSC
