The GPAT Program Information Form
Each program that plays on GPAT23 must have a separate Program Information Form (PIF) filled out and submitted. By filling out and signing a PIF, the GPAT Local Producer indicates that he or she has read and is following the GPAT Rules about submitting programs.
The form should be filled out and submitted with the program. Please do not tape or staple the form to the media.
If the form is submitted via email, as an attachment, the email message subject line should say: "Name of Show debut:12/12/12"
NUMBER ON FORM / EXPLANATION
1. Producer ID: / This is your producer ID, assigned by GPAT. If this is your first submission, you should also submit a producer information sheet. In such a situation, write "applied for."
2. Date submitted: / The date you delivered the program to GPAT
3. Is this program a one-time submission or is it part of a series that is regularly sheduled? / You must check either #4 or #5.
4. One time submission. / Check this box unless your program is part of a series. If you have not made previous arangements with GPAT, your program is not part of a series. Do not fill out #6, #7, #8, #9. -- Go directly to #10.
5. Part of a series. / Check this box ONLY if your program is part of a series. Then fill out #6, #7, #8, #9
6. Series Title: / This is the name of the program as it appears every week in GPAT regular schedule.
7. Program Title: / This is the name or content of this one individual program, that distinguishes it from other programs in the series.
8. Date this program should run for the first time. / This is the premiere date of this program.
9. This program should run / Check the appropriate box to indicate how many weeks this program should run on GPAT.
10. How are you submitting this program? / There are only 2 ways to submit a program to GPAT: A DVD video or as a MPEG2 digital file. Select #11 or #12.
11. DVD video
SKIP TO #16 / Check here if your program will play on regular DVD players.
Your program should be free of abnormalities that make the picturejump, skew, digitize, disappear, or in some other way not play properly. Audio levels must be consistent and be free of abnormalities that make the audio beep, screech or consist of other sounds that interfere with the intended soundtrack. Audio can be in stereo or mono, although it will be cablecast only in mono.
Each video program should be submitted on a separate DVD.
A DVD video should NOT automatically start playing after being inserted into a DVD machine! It should have a menu, with a single play command that will start the playback with 10-seconds of black (pre-roll time) before the program starts and 10 seconds of black after the program ends.
The following information must be written on the actual DVD disc. Information may be written or printed on a label that adheres to the actual DVD disc.
1. Producer ID
2. Program Name
3. Pre-Roll length (length of time from play to start of program)
4. Duration of program
5. Other information that the producer wants to include, such as title, or subject.
Media not properly labeled will not be scheduled.
Producers can submit their programs as MPEG-2 files on a DVD, via portable USB storage device,or the file could be transferred to GPAT via the internet, using a cloud-based storage system.

Programs submitted as a MPEG2 file should not have any pre-or post-roll time (i.e. no black before the program starts.)
12. Digital File on DVD media / Check this box if you are submitting your file on DVD media. Make sure to label the media with your Producer ID number, the Premiere Date of your program and the file name. File name should contain NO SPACES and no more than 27 characters. Suggestion for file name:
First 3 characters are producer number
followed by a hyphen
Next 6 characters are the date that the program be first cablecast. (Month,Day,Year: 070413)
followed by a hyphen
Next 3 characters are a 3-letter abbreviation of the series or program name
It is not necessary to use all 16 characters.
13. Digital File on portable media, such as flashdrive / Check this box if you are submitting your file on a USB drive. Make sure to attach a label the media with your Producer ID number, the Premiere Date of your program and the file name. File name should contain NO SPACES and no more than 27 characters. Suggestion for file name:
First 3 characters are producer number
followed by a hyphen
Next 6 characters are the date that the program be first cablecast. (Month,Day,Year: 070413)
followed by a hyphen
Next 3 characters are a 3-letter abbreviation of the series or program name
It is not necessary to use all 16 characters.
Multiple programs can be placed on a single thumb drive (USB drive), but make sure to name them properly and note the names on the forms to avoid incorrect airings.
14. Digital file via cloud and internet. Name of cloud storage: / Check this box if your are submitting your file via the internet. Be sure to provide all information that will be necessary for GPAT to download your file. You must inform GPAT that you have posted your program by sending an email to with attached form or links. Email subject line should say: "Name of Show debut:12/12/12"
15. If digital file what is file name? / Write the file name of your program. File name should contain NO SPACES and no more than 27 characters. Suggestion for file name:

16. Do you want to pick up your media? / Check yes if you want GPAT to save your media. You must arrange a time with GPAT to pick up your DVDs and/or other media. We will save your media up to 100 days.
17. Program Length / This should be an accurate duration of the program, not an estimate. This should not include pre or post roll black.
18. Does this program contain religious content? / Check yes if your program contains religious content. (opinions, discussions, teachings or performances that relate to or manifest faithful devotion to an acknowledged ultimate reality or deity.) Religious programs are cablecast on GPAT23 on Sundays and Wednesdays.
19. Was program produced in PittCounty? / Check yes if your program was created within PittCounty but outside the city limits of Greenville.
20. Was program produced in Greenville? / Check yes if your program was inside the city limits of Greenville.
21. Does program contain images of PittCounty or its Citizens? / Check yes if your program contains images of PittCounty or its citizens outside the city limits of Greenville.
22. Does program contain images of Greenville or its Citizens? / Check yes if your program contains images of citizens who live within the city limits of Greenville.
23. Does this program contain language or words or a word that are usually not accepted as appropriate for general audiences of any age? / Please contact the GPAT office if you questions regarding rules about language, words or images.
24.Does this program contain images that may violate community standards regarding obscenity / Please contact the GPAT office if you questions regarding rules about language, words or images.
25. Please describe contents of program, such as location, date recorded, names of people who appear in program. / This for our records and may be used in promoting the program on the GPAT website, on Facebook or printed publications.
26. Statement of Compliance By signing below you agree that 1) you have read and 2) understand and 3) are complying with the GPAT Rules about submitting programs. / All forms should be signed by the local producer or his/her agent.
By signing this form the producer indicates that he/she has read and understands GPAT Rules and that this program complies with the GPAT Rules about submitting programs. The GPAT Rules include that A. Programs are free of advertising or other material that promotes a commercial product, service or lottery
B. Programs are free of unlawful use of copyrighted material
C. Programs are free of material that violates local standards relating to obscenity
D. Programs are free of material that is libelous, slanderous or defamatory in character
E. Programs are free of material that is an unlawful invasion of privacy
Local Producer Signature: / If Program Information Form is submitted digitally, via email or cloud, producer should enter text: "Digital signature by (Producer's Name)." When sending email to with attached form or links, mail subject line should say: "Name of Show debut:12/12/12"
Date Signed: / This should be the date that program was submitted and/or the form was signed by the local producer.
27. This form is being submitted with the media. Y/N / Check Yes if the PIF and the program media are being submitted together, at the same time. Check No if this form is being submitted separately from the program, for example, if you are sending the PIF via email while dropping a DVD into the slot at the GPAT studio.
