Trainer’s Log Book

This logbook was developed by Bruce Davies, C.O at Northallerton after suggestions from some other Trainers’ Workshops in the Deanery.

The purpose of the Trainer’s Logbook is to enable the trainer to keep a record of the GP TRAINEE’s progress and the training that has been offered. The recorded evidence may also assist the trainer in completing the trainer’s report for summative assessment.

The first section is intended to enable the trainer to keep a broad overview of the Trainee’s training and background e.g. social factors can be used to note issues in the Trainee’s life which may impinge on training. The headings are meant as reminders of issues to be considered.

Please feel free to photocopy extra copies of those pages you find useful.

One idea of how to use this record is to incorporate it into your Trainee’s Logbook, so you both use the same document. This may foster your Trainee’s use of the Logbook. If you do this it would be imperative to keep a copy of “your Part” when your Trainee moves on.
