This form should be completed by anyone wishing to propose the establishment of a new scholarship or studentship or other similar award.

  1. Where are the funds for the scholarship coming from? What is the total value of the scholarship donation or funds available? Please provide the budget code from which the scholarship will be made (including a 5 digit expense code, e.g. ZAN-Z123-84069). Failure to provide this could delay the setting up of the scholarship

  1. What is the proposed value of each individual scholarship?

  1. Is the scholarship for fees or living costs or both? Please specify any other allowances covered

  1. Is this scholarship eligible for Gift Aid? If yes, please specify how those funds are to be used.

  1. How will the scholarship be publicised?

  1. Will the donation cover the full amount required each year? If not, where are the matching or balancing funds coming from (e.g. PhD Partnership Scheme, academic department)?

  1. If the scholarship is being financed by a donor, please provide some information about the donor (e.g. interests, reasons for establishing the scholarship etc)
