  1. What was your company seniority number on this day?
  1. What were your dates of employment with ONA?
  1. What was your assigned working position on the ACFT (Pilot, Co-pilot, Flt. Engineer, Relief Pilot, Pilot occupying a jump seat-not working)?
  1. Do you know who the other working assigned pilots were, their names, phone numbers, email addresses---any information?
  1. Do you know the name of theCaptain, Co-pilot, Flight Engineer, Relief Pilots, Jump Seaters Occupying cockpit jump seats assigned to this flight?
  1. Do you know the contact for any of these individuals F/A (phone, address, email address)?
  1. Where did the working assigned pilots board the ACFT from (a hanger, bussed to tarmac, at the airport via Jetway, etc.)?
  1. How many hours before the flight did the pilot briefing take place?
  1. At what time?
  1. What was the scheduled T/O time?
  1. Any interesting stories take place during the briefing (anyone late, replaced on the flight because they could not make it, got sick, etc)?
  1. Did this FLT depart on time?
  1. When the door closed, and ACFT taxied for departure was their any delay, or did it go right to the RWY and start its take-off roll?
  1. Do you have any pictures of this event? Do you know anyone who does?
  1. Besides this event, do you have any pictures of yourself or other ONA employees in your ONA uniform?
  1. Besides this event, do you have any pictures of yourself, or other ONA employees together (on a layover, party, on the ACFT, company headquarters, training, simulators, etc).

  1. Why did you pick/bid to do this trip?
  1. How long had you been an ONA employee?
  1. Was this your first time doing something like this?
  1. Had you been to the Middle East before and done Hajji flights?
  1. Was the DC-10 going fast when this happened?
  1. Did you hear loud / strange noises?
  1. From full throttle till abort, how long did that take?
  1. After the abort, how long before the EVAC took place?
  1. Who initiated it?
  1. Did the order to EVAC initiate from the working cockpit crew or working F/A’s?
  1. When your time came to jump out of the burning ACFT were you scared?
  1. Did you escape the ACFT via a cockpit window or escape slide?
  1. Did you pause and look outside before jumping and see the ACFT on fire?
  1. Was there pushing shoving on the ACFT?
  1. Was there screaming yelling?
  1. Did you smell smoke inside ACFT or see a fire inside the ACFT?
  1. With all the F/A’s and pilots onboard, was there confusion before the EVAC between the 2 groups on what to do?
  1. Do you know who the last employees were to exit the ACFT?
  1. Did you get hurt at any time, or know of anyone getting hurt?
  2. After everyone got off the ACFT, where did everyone go? How long were you there?
  1. Did you continue as an ONA pilot or resign after the accident?
  1. After ONA ceased operations did you fly for another company?
  1. If so, what airline, how long, what ACFT type, etc…
