If you, or someone in your family, are managing a chronic condition – diabetes or asthma, for example – we have important news. The ACEC Life/Health Trust is proud to introduce our Health Management program, a valuable addition to your benefits package provided at no additional cost.
Any insured individual who has been diagnosed with diabetes, congestive heart failure, coronary artery disease, asthma or COPD (chronic obstructive pulmonary disease) is eligible to receive this new benefit. The program provides 24/7 access to a team of registered nurses, dieticians, educators and pharmacists, all of whom work together and with you and your personal physician, to help you better manage your condition. Whether you have a question about how to take a medication, need support in making lifestyle changes, or are simply seeking some reassurance in the middle of the night, help is there every time you need it.
Education is an important component of the plan. If you are newly diagnosed, your personal health consultants will help you learn about your condition, and help you understand and follow your doctor’s treatment plan. Members who are already managing conditions benefit, too. The program provides access to the latest medical findings and literature, so all participants can become more knowledgeable about their condition, and optimize management of it.
