I hereby read, understand, and agree to the following listed:

1.  I understand and acknowledge that I assume all risk of accidents, injuries or even death which may result from or in connection with my participation in the 5th World Traditional Wushu Championships;

2.  I agree to hold harmless, waive and release the organizing committee (IWUF and other Host Organizations and Organizers) from any responsibility, liabilities, demands or claims of any kind by reason of any matter, cause or thing whatsoever that I may sustain as a result of or in connection with my participation in the 5th World Traditional Wushu Championships ;

3.  I promise that I have not taken any dope or narcotic drug;

4.  I guarantee that I have not involved in or been suspected for any illegal activity;

5.  I acknowledge that I am physically fit and mentally capable of taking part in the competitions;

6.  I will bear full responsibility of taking care of my personal property and valuable items. Neither the host nor the organizer shall be held responsible for any loss, steal, or damage occurred in the competition field;

7.  I fully understand that all medical attention or treatment afforded to me by the Organizing Committee its officers, representatives, volunteers, and all other related members will be of the first aid only, and hereby release the Organizing Committee and all other related members from any liability for such aid;

8.  I agree that my performance, attendance, and participation in the 5th World Traditional Wushu Championships may be filmed or otherwise recorded or released or telecast live. I consent the Chinese Wushu Association to use my name, address, voices, poses, pictures and biographical data concerning full or parts, in any form or language, with or without other material, throughout the world, without limitation, for television, radio, video, theatrical medium picture, or any other medium by any devices now known or hereafter devised and I do hereby waive any compensation in regard thereof as well as any future rights to the aforementioned..
