SMART Moves manuals – can obtain via Boys & Girls Clubs of America eStore, must follow curriculum (Start SMART, Stay SMART, SMART Leaders), see Kevi for additional resources, administer SMART Moves Pre/Posttests & keep in client files

Form 2970c & Form 2971c – DFPS background check forms that each staff member who will have direct, constant contact with TEXSYN youth and/or their client files must submit to the Alliance office every TWO years

Fast Pass – form used to schedule FBI fingerprinting, required for staff who have lived outside of Texas within the past five years

SYSN Registration Form – each participating child must complete, parent sign, staff sign, & submit to the Alliance office (submitted via mail only)

DFPS Pre/Post Test – each participating child must complete pretest prior to start of SMART Moves sessions, posttests will be administered ONCE per year to be used as a bench mark for their progress while participating in program, copies of tests will be submitted to Alliance office (submitted via mail only)

ISP (Individual Service Plan) – each participating youth should have a copy of ISP placed into their client file & updated monthly, used to track participation in all programs at Club (do not have to submit to Alliance unless requested)

Case Notes – each participating youth should have a copy of the case notes page placed into their client file & updated monthly, TEXSYN staff should write personalized notes about the child (do not have to submit to Alliance unless requested)

MSP (Monthly Services Provided) – used to report monthly attendance numbers for BGC Experience and SMART Moves sessions (submitted via email)

Inactive Request Form – used to report youth who were registered but have not actively participated for 3 or more months (submitted via email)

Quarterly Report – submitted 3 times per year, used to tell DFPS about all activities going on at Club (submitted via email)

Annual Report – submitted at end of fiscal year to report on program for the year (submitted via email)

Fidelity Instrument – tool that programs should be based upon, can be found in your SMART Moves manuals

SMART Moves Pre/Post Tests – curriculum based pre and posttests, available for Start SMART and Stay SMART, should be administered to TEXSYN Participants, evaluated and kept on file

DUE DATES

Background Check Forms – submitted to Alliance office PRIOR to contact with TEXSYN youth, every two years

Registration Forms & Pretests – due on the 20th of each month (example: if you will be including on September MSP, must be received by the 20th of September)

ISP & Case Notes – updated monthly, kept in client files, do not have to submit to Alliance unless specifically requested

MSPs – due by the 5th of every month to (example: you will serve kids in September & report their September services by October 5th)

Quarterly Reports – due to by December 9th, March 10th, June 9th

Annual Report – due to by August 18th, 2017
