Theinformationprovidedbysupervisorsisused:

•toprovidejuniordoctorswithfeedbackontheirperformanceandindicateareasforprofessional development

•tocontributetoMedicalBoarddecisionsonregisteringinterns.

Theinformationprovidedbyjuniordoctorsisusedtopromotediscussionbetweenjuniordoctorsand supervisorsatmid-termandend-of-termassessmentmeetings.Juniordoctorcompletedformswillnot becollected.

Instructions for Junior Doctors

Youshouldcompletethisformindependentlybeforethemid-termandend-of-termassessment

meetingsanddiscussitwithyoursupervisoratthosemeetings.Considerbothyourstrengthsandareas whereyoucouldbenefitfromadditionalexperience.

Instructions for supervisors

Thisformmustbediscussedwiththejuniordoctor,andthediscussionshouldincludethejunior doctor’sself-assessment.

Term supervisors should:

1.Observethejuniordoctorintheworkplacebeforecompletingthisassessment.

2.Includeobservationsfrommultiplesources(e.g.othermedicalpractitioners,nurses,otherhealth professionals,patients).

3.Tickappropriateboxesintheperformanceratingssectionbyreferringtotheperformanceratingkey.

Everyeffortshouldbemadetoobservethejuniordoctor’sperformancetoallowassessmentagainst eachoutcomestatement.Ifthisisnotpossible,theoption‘Notobserved’shouldbeticked.

4.Userating2andrating4wheretheperformancefallsbetweenthedescriptors.

5.Complete(atthemid-termassessment)theImprovingperformanceactionplan(IPAP)sectionforeachitemwherethejuniordoctorrequiressignificantimprovement.AnIPAPisrequiredforall

ratingsof1or2,andfor3insomecircumstances.CompletingtheIPAPshouldincludediscussion withthejuniordoctorandtheDirectorofClinicalTraining(orequivalent)toensureremediation

commenceswithclearlyarticulatedgoals,timelinesforreview,andactionstobetaken.TheIPAPshouldbereviewedattheend-of-termassessmenttodetermineifremediationiscompleteorif

ongoingactionsarerequired.

6.Decideattheend-of-termassessmenttheglobalrating(overallperformance)forthejuniordoctor,consideringtheirabilitytopractisesafely,toworkwithincreasinglevelsofresponsibility,toapply existingknowledgeandskills,andtolearnnewknowledgeandskillsasrequired.
