Student Education Induction Pack
This induction pack is to help you as a learner to familiarise yourself with the team. It provides you with the basics about the team and should help you to plan to meet your learning needs.
We recommend that all learners contact the team in good time prior to placement to identify who will be supervising your learning and confirm start times/hours.
During your placement you should have a minimum of a preliminary, midpoint and final meeting with whoever is supervising your learning. You should also undertake some form of leaning analysis then produce a learning contract and or action plan that provide structure and direction to the time you are with us. Your supervisor will be able to help draw this together but you must take a lead in negotiating your plan. Some examples of these tools are provided but you may wish to use those produced by your education provider.
Please ensure that you present any documents regarding your placement with our staff on the first day of your placement. This should include any assessment document, learning outcomes you hope to achieve and any type of ongoing record of achievement.
On completion of your placement you will be asked to provide an evaluation. Please take time to complete this openly and honestly as it is only with your feedback can we improved the quality of these valuable learning experiences.

Service Description: / Telford and Wrekin Crisis Resolution Home Treatment Team provide an adult acute mental health needs led assessment and intervention service which offers effective and therapeutic treatments in the Telford and Wrekin community as an alternative to hospital admission.
Team philosophy: / Telford and Wrekin Crisis Resolution Home Treatment strive to provide a realistic alternative to hospital with minimal disruption to the lives of those who use the service and that of carers/family & friends. Clients choice is a primary consideration and the team encourages people who use the service and their carers/family/friends to be actively involved in the planning, implementation and review of the care they have from the team. Our aim is to respond in a prompt and flexible manner to referrals from our colleagues in Telford and Wrekin. We encourage close working with care co-ordinators and other providers of mental health care to meet the physical, psychological, social, cultural and spiritual needs of individuals and families, safely within their own home environment. To work with people who utilise the team their carers/family/friends to develop the service that we provide and plan for future improvements in service delivery.

Expectations of students / Students are expected to work within their discipline’s code of practice and Trust policies. Be punctual in their attendance. Informing the team and the university if unable to attend placement. Exchange of contact details. Feeding back concerns and problems to their mentor. Providing their mentor with principles of good practice, clinical inventory, and attendance record. Providing their mentor with the continuous assessment of clinical practice document on summatively assessed placements. Providing their mentor with their pre-registration portfolio. Supplying and undertaking learning contracts, SWOT analysis and action plans. Facilitating own reflective practice. Completing the placement evaluation at the end of the placement. Dress & Appearance appropriate to the allocation in line with university & trust policies.
