2013 FHSAA Flag Football Finals

Team Data Form for Official Souvenir Program & Press Booklet

Schools whose flag football teams advance beyond the District Tournament must complete and return this three-page team data form to the FHSAA Office via e-mail () not later than 5 p.m. ET Friday, April 26, 2013.

MANDATORY ELECTRONIC TRANSMISSION: This team data form must be completed and transmitted electronically via e-mail to “”. A school that fails to meet the deadline shall be assessed a $50.00 late fee. If the school advances to the FHSAA Finals and the materials are not received by the FHSAA Office’s publication deadline and, therefore, do not appear in the souvenir program, the school shall be assessed additional fines up to $300. SHOULD A SCHOOL NOT CALL TO CONFIRM RECEIPT OF THESE MATERIALS, THE FHSAA IS NOT RESPONSIBLE FOR THE LOSS OR NON-RECEIPT OF THESE MATERIALS.

MANDATORY SUBMISSION OF UP-TO-DATE STATISTICS: The eight schools advancing to the FHSAA Flag Football Finals must report up-to-date cumulative statistics through the FHSAA Finals Second Round Play-In Game by completing and returning the “2013 FHSAA Flag Football Finals Up-To-Date Team Statistics” form, which is available for download on the FHSAA web site, to the FHSAA Office via e-mail () not later than noon ET Wednesday, May 1, 2013. Each qualifying school that does not meet the deadline shall be assessed a $100 fine.

Instructions on Using This Microsoft Word Form

1. Save and name this form according to your school name and classification. For example, Miami High School would name its form "miami.doc". Wesley Chapel School would name its form "wesley_chapel.doc". Windows users are reminded that you may not be able to exceed eight characters in the main file name preceding the .doc suffix.

2. Type in all of the information requested on the form. Type the information into the white fields. Avoid typing in the gray highlighted areas. Avoid causing one of the white fields to break or expand beyond the space allotted. Move from table to table in the form using your mouse and cursor. Move from field to field in each table using the "tab" key.

3. Double-check and triple-check all information, especially the team roster information. The information you provide will appear in the official souvenir program exactly as you provide it.

4. Save frequently as you complete the form to avoid losing your work.

5. Once you have completed the form, save it. Print a hard copy for your files.

6. To file the completed form with the FHSAA Office, you must:

(a) E-mail the Microsoft Word file as an attachment to "". In the subject field of the e-mail, type in the name of your school, the sport and word "form" – also include the words “NO SPAM” in the subject line to avoid having your email get caught up in the spam filter. For example, South Lake High School would type in "South Lake flag football form NO SPAM" in the subject field; and

(b) Call (352) 372-9551 ext. 190 and confirm its receipt in the FHSAA Office. If you receive an e-mail response confirming receipt, YOU DO NOT HAVE TO CALL, but please keep a copy of the response on file for your records.

7. Just in case we haven’t emphasized this point enough, you must call (352) 372-9551 ext. 190 and confirm receipt of your team data form in the FHSAA Office if you do not receive confirmation via e-mail. SHOULD A SCHOOL NOT CALL TO CONFIRM RECEIPT OF MATERIALS, THE FHSAA IS NOT RESPONSIBLE FOR THE LOSS OR NON-RECEIPT OF THESE MATERIALS!!!
