Cottonwood Heights Adult Soccer League is hosting an open invitation 3 V 3 soccer tournament for charity. The tournament will be held Saturday June 20th. Full team registrations will be accepted first and if the 16 team spots are not filled, individual registrations will be accepted. Each participant must complete an individual registration form, whether they are a part of a team, or registering as an individual. The registration fee is $20.00 per player, and this includes a T-shirt, and a guarantee of a minimum of 3 games. This is a coed tournament format and at least 1 girl needs to be on the field at all times. Each team can have either 6 -8 players, and each player must register and pay the $20.00 to participate. Team registrations are due by 6/6/15. Individual registrations will be accepted on 6/7/15 until the 16 team spots are filled. If you register as a single player you will be placed on a team by tournament organizers. The registration deadline to participate is June 12th. The minimum age to participate is 14 years old as of July 31st 2014.

Tournament Risk Waiver and Release of Liability:
I acknowledge that soccer and soccer tournaments are extreme tests of a person’s
physical and mental limits and that my participation in a soccer event can cause potentialdeath, serious injury, or property damage. With a full understanding of the potentialrisks, I HEREBY ASSUME THE RISKS OF PARTICIPATING OR OFFICIATING INA SOCCER EVENT. I hereby take the following action for myself, my executors,administrators, heirs, next of kin, successors and assigns:
a) I WAIVE, RELEASE, AND DISCHARGE Cottonwood Heights Adult Soccer League, its
organizers, referees, associates and field owners from any and all claims or liabilities fordeath or personal injury or damages of any kind, which arise out of or relate to my
traveling to and from or my participation in the Cottonwood Heights Adult Soccer League;
b) I AGREE NOT TO SUE the Cottonwood Heights Adult Soccer League, its organizers,
referees, associates and or field owners for any of the claims or liabilities that I have
waived, released or discharged herein;
c) I INDEMNIFY AND HOLD HARMLESS the Cottonwood Heights Adult Soccer League, itsorganizers, referees, associates and or field owners from any claims made or liabilities assessedagainst them as a result of my actions.
