5. Your email address (both USF and personal), local U.S. mail address, and telephone. This information is vital in case of urgent communications regarding grades, missing assignments, and similar matters.

6. Your writing experience and professional goals. Describe likes and dislikes, needs, challenges (such as using English as second language, working full-time while attending school, caring for dependents at home, health issues), and personal objectives. In short, provide any information that may help me meet your personal and academic expectations in the course.
