SWCD CAPACITY GRANT APPLICATION CHECKLIST
Oregon Soil and Water Conservation Districts
July 1, 2017 - July 31, 2019
The Soil and Water Conservation District (SWCD) must submit by email to the Oregon Department of Agriculture (ODA) the following forms and documents for the 2017-2019 Capacity Grant:
1. SWCD Grant Application Form: All sections of this form must be completed and submitted with all other application materials as listed below.
2. Racial and Ethnic Impact Statement Form:
3. Scope of Work and Focus Area Action Plan: Complete the required sections in the Scope of Work and the Focus Area Action Plan. SWCD’s using Monitoring Task must submit the ODA approved monitoring plan.
4. Capacity Grant Financial Report: Complete both columns “Estimated Budget” outlining expenses to be charged to the Scope of Work or the District Operations budgets for the 2017-2019-grant cycle. Submit signed original with grant application materials. The budget categories listed on the Expense Form must match the categories provided. Subcategories may be used to provide more detail on the estimated budget and reporting expenses.
Other Requirements due to ODA:
a) Long Range Plan (LRP): Must have a current LRP on file with ODA (ORS 568.554).
b) Submit annually a copy of the Report to the Secretary of State: 1) In Lieu of Audit Report, 2) a Financial Review, or 3) Full Audit (ORS 568.610 and ORS 297.435).
c) Annual Work Plan (AWP): Submit the SWCD Board approved AWP for each fiscal year (ORS 568.554).
d) Annual Report: Submit each fiscal year a copy from presentation at the Annual Meeting (ORS 568.580).
e) Annual Meeting and Notice: 1) Submit annually the board Resolution setting the Annual Meeting date, and 2) Copy of the newspaper public notice published no later than 15 days prior to the Annual Meeting (ORS 568.580 and ORS 568.950).
