Supporting Documentation Checklist
Infrastructure Applications
For FULL applications seeking up to $300,000 please submit the following documents with your application(where relevant):
□Floor and site plans for the activity / □Evidence/projections of the growth of participation generated by the activity□Planning Permits, building permits and approvals (or an indication of their approval date) / □Evidence of land ownership or a long term lease (if relevant)
□Soil test report / □Quotes or qualified cost estimates
□A project plan showing key activity steps, milestones and time frames / □Information on Environmentally Sensitive Design (ESD) and Universal Design features of the activity and confirmation of compliance with Australian Standards
□Voluntary Labour and In-Kind Sheet (where in-kind contributions are included in the project budget) / □Letters of support for the project
□Evidence confirming all non LVA project income sources / □Copies of the two most recent annual reports/financial statements from your organisation
□Evidence of Council, DEWLP, Parks Vic or land owner endorsement
For FULL applications seeking more than $300,000 or where the project exceeds $500,000 in total cost, please submit the following documents with your application where relevant:
□Evidence of job and economic outcomes being provided by the activity / □An evaluation plan, report and audit report will be required for large scale projects. Audit reports not required for local government projects.□Professionally prepared cost estimates, e.g. Quantity Surveyor, engineering or construction estimates / □A risk management plan
□Schematic Plans for the project (desirable) / □Letters of support from at least two relevant organisations that clearly define their involvement with the project and the anticipated benefit falling out of the project
□Evidence of life cycle cost management and capital replacement plans / □A copy of the two most current annual reports including audited financial statements and the most recent monthly report or a link to a website where this information is available (not required for Local Government Agencies)
□Evidence of governance and activity programming arrangements / □Joint use agreements, letters of intent or MOU’s where relevant to the activity
It is anticipated that projects of this size would be managed by organisations with a strong track record of similar projects or Local Government.
Event Applications
For FULL event applications, please submit the following documents with your application:
□The full budget for the event
□Letters of support for the event that outline the community benefits
□Voluntary Labour and In-kind Sheet (only where this is being used to generate LVA matching funding)
□Evidence confirming the event income (e.g. LGA funds, fund raising, philanthropic contributions)
□Evidence of Council approval
□Evidence of approval by Victoria Police, permits, licences (if required)
□Copies of the two most recent annual reports/financial statements of your organisation (or link to your website where the information can be found)
□A full project budget, with income equaling expenditure
