New Supplier FSE EHS Program and Training Evaluation

PART 1: PURPOSE AND EXPECTACTIONS

Purpose:

The purpose of this evaluation is to ensure a "new to Intel" supplier has the appropriate EHS programs and training in place prior to sending field service support personnel to Intel to conduct equipment maintenance.

Expectations:

·  Form is filled out and returned to Intel EHS upon request

·  For training/programs identified as required, Intel EHS will request proof of documentation (e.g. training curriculum and written programs).

·  Intel EHS reserves the right to request proof of training for any field support personnel that may come onto an Intel site

·  If there are any training or written program gaps, a corrective action plan must be documented by the supplier and all action items closed prior to Intel allowing field service personnel onto an Intel site.
