Please answer the following questions (except question 1) in no more than two paragraphs each.

1)  List all of the undergraduate and graduate level mathematics courses that you have taken. Include name of course, the semester and year taken, the name of the college or university where you took the course, and the grade you received. Please also list Spring 2017 courses which are in-progress.

Note that we will travel as a group to MAA Mathfest in Chicago July 26-29/30; you may either return to Amherst at the conclusion of Mathfest, or fly directly to another location from Chicago, as this summer research program ends at the conclusion of Mathfest.
