*Minor Changes. Form 30’s raised to notify the TAA of Classification of Minor Changes should complete sections 1, 3 and 6 only. In this instance, references to the applicant may be a DO under Privilege, or LOAA holder.

Form 30 Application for Military Type Certificateor Change in Type Design & Notification of Minor Changes to the TAA

Submission of a completed Form 30 enables the MAA to process applications without undue delay.

Please Note that only a Single Air System Application request will be accepted per each Form 30 submitted, this is to enable an efficient audit trail to be maintained.

Project Security Classification/Handling Caveats / Enter the Project Security Classification as stated in the Defence Policy and Business New Government Security Classification System (GSC)including National Caveats as appropriate as the Form 30 is classed only as Official.
1 / The applicant is the TAA or LOAA post holder empowered to endorse DO Certification.
1.1 / Enter your reference (if applicable) and/or Project File Reference (the Form 030 should be maintained as part of the Airworthiness/Engineering file for the Type change and forms part of the Certification Strategy).
1.2 / Enter the full name of the TAA or Delivery Team (DT) making the application.
1.3 – 1.6 / Enter TAA and DT Certification Manager Names,complete registered TAA and DT address, full telephone number (full GPTN and Civil codes), and e-mail addresses.
The DT Certification Manager is the project desk officer responsible for the delivery of the certification work stream. This is the person the MAA Cert Team will treat as the primary POC for MTC progression.
2 / State and give a brief description of the Air System type, Mk and model applied for (include Air System description where details are pertinent to the change being considered or the project is introducing a New Military Type).
2.1 / Engine/Propeller/APU Manufacturer – Design Organization and if different OEM

3 / For the TAA or the DO under privilege[5] to specify the classification iaw RA5810 and RA 5820.
Note: Minor changes are processed under TAA assurance iaw RA 5850. There is no requirement for Minor modifications to be sent to the MAA on a Form 030.
3.1 / A description of the project and identify the systems likely to be affected by the change. If applicable, make further reference to drawings, basic data, operational characteristics/limitations as required, high level features, technologies,airworthiness codes. This should reflect the project description within the Type/Air System Airworthiness Strategy. For Modifications this should reflect the “Background” section of the project Certification Strategy which should accompany a Form 030 submission.
3.2 / The justification should summarize the nature of the change to the Type Design and the basis for determining the classification, continue on a separate sheet if required. An explanation of which condition listed in RA5820 has driven the classification to Major. An explanation by the TAA or an appropriately approved DO under privilege5 has approved the classification as Minor iaw RA5820. If certifying a new air system, leave this field blank.
3.3 / Design Organisation (DO) reference Number is awarded by the MAAs DAOS branch to the DO following successful navigation through the DAOS process.
3.4 / For a new Air System, the Applicant should state the intended Design Safety Target and use the ‘Explanation’ field to give their rationale with reference to the AMC to RA 1230. For Major Changes, the Applicant should state the current Air System Design Safety Target. The ‘Explanation’ field should detail the effect of the change on the anticipated achievement of the Design Safety Target.
Note: Section 3.4 does not need to be populated for Minor changes.
4 / For a new type section 4 should contain reference to any existing Military Type Certificate from other approving authorities or Civil Type Certificates.
For a modification this should contain the Military Type Certificate reference for the Air System and any supporting civil type certificates. Where a MTC has not been issued provide the reference to the Air System specification and Certificate of Design and Configuration Status Record. This should include reference to existing equipment certification that the modification intends to introduce. Continue on a separate sheet if required.
5 / The Key Milestone and Proposed Dates should reflect the dates that the project intends to meet key milestones. The application will be rejected should dates be ommitted without appropriate justification.
The MAA will allocate resource to project milestones as appropriate subject to MAA resource availability. Should there be insufficient MAA resource for the dates proposed, the MAA will advise the TAA in our response, advising when resource is available to undertake the activity.
The MAA will accept dates as quarters (Q3 2019) if the project is at an early stage. However, these dates should be refined and communicated formally via a letter from the TAA or DTL/DM* to the MAA allocated PCM at the earliest opportunity.
If the proposed dates are later amended, this should be communicated formally via letter from the TAA or DTL/DM* at the earliest opportunity, addressed to the MAA allocated PCM, PCL or Cert-DepHd.
*When the DTL/DM sends a formal proposal to amend key milestone dates, the TAA must be included in the correspondence distribution list. MAA resource will not be commited to a milestone if the TAA is not included in the correspondence.
6 / Signature of an authorized representative of the applicant (TAA representative, Delegated LoAA Holder, DO under privilege etc) and ensures a copy shall be sent to the OC Safety & Engineering Team or equivalent.
When classified by a DO under privilege, the signatory must be approved by the DO iaw RA5850.

[1] The TAA must be the applicant for Major changes, Mark number changes and new Air Systems.The LOAA or a DO under privilege may be the applicant to the TAA for Minor changes.
