Submission Form

Name ______

MHC Address: ______Email address: ______

Home Address: ______

Title: ______

If the work is not written, what are you submitting? (e.g., videotape of a performance, sculpture in the Student Art Show, etc).

If your submission began as a course assignment, please give the course and instructor:

If your submission is a thesis (or portion of a thesis), please give the Department and Program and Advisor(s).

Please provide a short rationale for your work. Some projects are going to require expert knowledge to understand – please let us know to whom you addressed your work. Other projects are accessible to a much larger audience, but often that audience would be better readers of your work if they knew something about the context into which it fits, and what brought you to it. You might want to point readers to particular sections that give a good sense of your work. Please attach your comments to this form.

The following applies to some projects. If it applies to yours, please discuss. Some student projects are also parts of a larger class effort, or part of a large team effort, or involve, say, directing a play. If so, please discuss the relation of your contribution in relation to that larger work.

Feel free to append further comments that you believe would be helpful to others on meeting your work.

Submit form to the Biology Department Office, 104 Clapp Lab, by 11:00 p.m. on Monday, May 4.

All submitted works can be picked up in the Biology Department Office on Saturday, May 23, between 2:00 – 4:00 p.m., or on the proceeding Friday.
